• Provide high-quality individual therapy to a diverse client population, using evidence-based modalities tailored to each client's needs • Conduct thorough intake assessments, develop individualized treatment plans, and adjust care as clients progress • Maintain a consistent caseload with strong retention, booking clients 3–4 weeks in advance to support continuity of care • Complete timely, compliant documentation, including session notes, treatment plans, and insurance paperwork by end of day • Collaborate with colleagues, care coordinators, and referring providers to deliver integrated, whole-person care • Deliver services via telehealth and/or in-office, maintaining a private and professional environment in either setting • Communicate proactively with clients about scheduling, policies, and treatment expectations • Participate in team meetings, clinical trainings, and ongoing professional development • Stay current with clinical best practices, ethical standards, and licensure requirements • Contribute to a collaborative culture that prioritizes clinical excellence and sustainable clinician wellbeing
Job Requirements
- Active and unrestricted licensure as LMFT, LCSW, LPCC in good standing
- Demonstrated experience providing individual therapy services in an outpatient or similar setting.
- Experience working with electronic health record (EHR) systems and maintaining accurate clinical documentation.
- Ability to follow established SMART note documentation protocols.
- Strong time management, scheduling discipline, and organizational skills.
- Demonstrated commitment to ethical practice, confidentiality, and HIPAA compliance.
- Ability to manage an independent caseload while maintaining responsiveness and reliability for clients.

Role Description The Care Coordinator is a trained professional that helps patients manage their chronic conditions by calling patients monthly.
