The Complex Care Navigator supports patients and families living with serious illness by helping them navigate the healthcare system and align medical care with the patient's goals, values, and lived experience. This role serves as a consistent point of contact for patients and caregivers, fostering trusting relationships and helping bridge communication between patients, families, providers, and care teams. The navigator works collaboratively within an interdisciplinary palliative care team to support coordination, communication, and patient-centered care. Schedule: A 40 hour week, Monday through Friday from 8:00 am to 4:30 pm Location:7600 EVERGREEN WAY, Everett, WA 98203 Primary Responsibilities: - Enroll patients with serious or complex illness into the palliative care program, explain program services, and serve as a primary point of contact for patients and families. - Maybe the ability to conduct work in a hybrid setting, including approximately 70% office-based responsibilities and 30% community-based activities such as home visits and patient outreach. - Strong emotional intelligence and ability to navigate sensitive, emotionally complex situations and family dynamics. - Strong organizational skills with the ability to manage competing priorities independently. - Ability to work independently while remaining highly collaborative with healthcare teams. - Maintain confidentiality of patient information in accordance with HIPAA guidelines and Optum policies. - Attend required training and maintain compliance with state, federal, OSHA, DOSH, and organizational requirements. - Maintain regular and reliable attendance. - Performs all other duties as assigned What are the reasons to consider working for UnitedHealth Group?
