Refer patients to social services and community resources & follow up to ensure patient access to these supportive resources. Provide patients with health education and coaching to help them manage their diabetes and/or hypertension. Training / Certification / Licensure: Community Health Worker certification preferred. Lay navigation training certification will be considered as an alternative to CHW certification.
