Adult Psychiatrist, PMHNP
Community Counseling Solutions
• Provides psychiatric diagnosis, care and treatment on an in-patient and telepsychiatry basis and/or in person to improve the mental health of clients • Completes psychiatric evaluations as required on new clients • Reevaluates and updates as needed • Performs age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities • Implements interventions to support the patient to regain or maintain physiologic stability • Monitors the effectiveness of interventions • Facilitates the patient’s transition within and between health care settings • Reviews and evaluates clinical data pertaining to physical status of client • Reviews and follows up on previous mental and physical treatment plans • Leads prescribed individual, group therapy and/or education sessions • Reviews, approves and signs individual plans of service of clients • Documents and updates client’s progress in progress notes • Prescribes client medication • Monitors medication effectiveness and modifies as necessary • Works with team members to discuss client progress and outlook • Coordinates with outside agencies as necessary • Act as a client advocate when appropriate • Contacts client’s relatives to arrange family conferences if necessary • Provides after hours services during regularly scheduled work week • Responsible to work closely with supervisor to find backup when on vacation
Job Requirements
- Medical degree in psychiatry from an accredited program
- Completion of an Adult residency
- Licensed by the State of Oregon
- Graduate of an accredited school of nursing for PMHNP
- Graduate of an accredited Nurse Practitioner Program for PMHNP
- Current Nurse Practitioner certification in specialty area
- Individual must be able to pass a criminal history background check
- Preference given to individuals who have worked with individuals who have mental illnesses in a residential or psychiatric hospital setting

• Provide administrative support to the members of the CM team • Collect data for Health Risk Assessments (HRA) • Screen for eligibility and benefits • Identify members without a PCP and refer to Member Services • Screen members by priority for CM services and refer as appropriate • Perform transition of care duties to include, but not limited to, contacting the members’ PCP, Medical POA, or other medical providers for information pertaining to special needs • Document all encounters and contacts made on behalf of clients; complete and submit monthly reports; maintain comprehensive electronic member records • Motivate members to be active and engaged participants in their health and overall well-being • Provide educational promotion, member follow-up, arrange PCP visits, and perform care coordination under the direction of the Care Manager • Assist members in assessing health-related services, including but not limited to obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care, and/or social services • Perform tasks as directed by the CM team to promote member compliance, such as verifying appointments or obtaining medical records • Facilitates member access to community resources, including, but not limited to, finding housing, food, clothing, prenatal classes, parenting, and providers to each life skill, and relevant mental health services • Work collaboratively and effectively within a team • Establishes positive, supportive relationships with participants and provides feedback to other members of the team • Builds and maintains positive working relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff, from diverse cultural and socio-economic backgrounds
• This position is responsible for conducting medical management and health education programs for customers on government health care programs. • Accountabilities include gathering, analyzing and providing date for regulatory reports. • This position will represent the company to members.
