Family Resource Specialist
Imagine Pediatrics
• Provide direct support to patients and families in addressing social needs • Work collaboratively with community partners and care team members • Identify resources and community support available to the family or caregivers • Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services • Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems and access needed services • Advocate with community agencies for, and on behalf, of clients and families • Assist in development of an internal community resource library • Connect with relevant internal staff and providers to assist with system navigation • Network with new and established community resources to increase knowledge of services
Job Requirements
- Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred
- Certification as a Community Health Worker highly desired
- Familiar with navigating social service agencies
- Must have reliable transportation, a valid Driver's License, and ability to produce a copy of current auto insurance certificate
- Experience with resourcing and telephonic services
- Bilingual Spanish preferred
