Virtual Care Referral Coordinator

• Assist members via telephone and messaging as part of a virtual primary care team to ensure that all external and internal referrals and orders such as specialist referrals, imaging orders, prior authorizations and other orders, are complete and properly routed to the appropriate external provider. • Assist patients with maps, facility location and direct dial numbers. • Coordinate medical records to ensure that all required records, imaging, and/or procedural results are available to the external provider before the patient's appointment and added to the internal medical record for provider review. • Assist in the management of electronic medical records and records requests • Assist providers in completing patient forms (i.e. Medical Opinion forms, SSI Disability forms) when appropriate. • Work collaboratively with staff including providers, the care team and patient support, compliance, etc. • Be able to handle a fast-paced dynamic environment with competing priorities

- Work experience in a physician's office or medical institution with exposure to managed care and primary care rules and practices.
- Ability to be self sufficient and use resources effectively to answer questions
- Ability to multi-task and juggle multiple priorities at once

• Conduct psychiatric evaluations and medication management visits for FamilyWell program participants via telehealth for the short-term management of mental health conditions in the perinatal period and/or experiencing peri/menopause. Provide treatment recommendations for patients who may need more intensive or more specialized mental health care. • Complete pre-conception medication consultation and psycho-education for individuals trying to conceive and currently treated with psychotropic medications. • Perform asynchronous caseload consultations through a review of clinical information prepared by the patient’s care team, as well as review of available medical records. Case review consultations focus primarily on patients who are new to the caseload or who are not improving as expected under their current treatment plan. Provide recommendations for next steps in treatment. • Complete documentation for case consultations and direct patient visits within 24 hours of visit. • Work with care managers to oversee the status and population health of their assigned patient caseload. The psychiatric consultant will use FamilyWell’s registry to track all assigned patients and prioritize patients for case review and treatment adjustment. • Provide the care team with educational support on diagnosing and managing behavioral health conditions. • Be available to provide occasional ad hoc telephone, email, or telehealth consultations to OB providers for complex patients or challenging patient situations within normal working hours. Maintain professional cell phone and email accounts for contact during usual business hours.
