• Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions. • Help patients execute personalized care plans and achieve their goals by providing education and coaching. • Conduct comprehensive assessments of patients' health status and readiness to change. • Educate patients on disease management, medication adherence, and prevention strategies. • Monitor patients' progress, adherence to treatment plans, and health outcomes. • Document patient interactions and identify barriers to care. • Collaborate in real time with clinicians and patient support teams.
Job Requirements
- An active Medical Assistant certification from an accredited association is required.
- At least 5 years of experience as a Medical Assistant is required.
- Remote work readiness (including stable high-speed WiFi) is required.
- Experience working in internal medicine, cardiology, nephrology or with the geriatric population is preferred.
- Experience providing patient education and support to individuals managing chronic conditions is preferred.
- Fluency in Spanish is strongly preferred.
- Fluency with AI-assisted clinical tools is required.

• Provide business support through Salesforce administration and development efforts, including requirements gathering, testing, and ongoing maintenance. • Manage and maintain data within Salesforce, ensuring accuracy, completeness, and compliance. • Facilitate business admin processes to include external data into Salesforce. • Develop and deliver Salesforce reports and dashboards to support TMs and leadership in sales execution. • Collaborate with multiple teams across IA Distribution and the enterprise, including leadership, distribution, data & reporting, ACM, and SFEA. • Gather, analyze, and translate business needs into technical requirements and admin-facilitated solutions. • Communicate effectively with business and IT partners, ensuring alignment and clarity on project goals. • Identify system gaps and process improvements, providing recommendations to enhance Salesforce functionality and business outcomes.
• Reviews Regulatory requirements (Statutory Plans) & translates to internal data reporting requirements. • Develops and prepares new reports and applications using multiple sources of information. • Evaluates information being reported for consistency, completeness, accuracy, and reasonableness. • Maintains and provides support for existing tools and applications used in the analysis of information. • Communicates with actuaries, accountants, product managers, and IT counterparts regarding data and business requirements for new reports or applications. • Prepares internal and external correspondence related to the development and understanding of information. • Conducts research into proposed solutions for data quality issues and the development of new reports or applications. • Implements process efficiencies and automations. • Assists in the cross-training and development of other professional personnel as assigned.
