Nurse Practitioner - Lifestyle & Longevity Medicine

Role Description Help us build the future of healthy aging. Total Life is a national telehealth platform built for older adults. Today, we deliver behavioral health care to Medicare beneficiaries across 49 states through a network of 200+ credentialed providers. This is a ground-floor role. You won't be slotting into an existing clinic; you'll be shaping one. If you've ever wanted to design how lifestyle medicine should be delivered to older adults online — the protocols, the patient journey, the standard of care — this is that opportunity. Important: this is not a primary care role. You are not the patient's PCP. Instead, you'll work alongside patients and their care teams as a dedicated lifestyle and longevity partner — focused on prevention, behavior change, and the daily habits that determine how well someone ages. What you'll do: - Partner with older adults to assess their health and wellness goals and build personalized, evidence-based lifestyle care plans. - Coach patients through sustainable behavior change grounded in lifestyle medicine — nutrition, movement, sleep, stress, connection, and reducing risky substances. - Anchor your practice in the 4 M's of Age-Friendly care — What Matters, Medication, Mentation, and Mobility. - Help shape the clinical model itself: protocols, intake, outcome measures, documentation, and the patient experience from first visit onward. - Collaborate closely with our behavioral health team to support the whole person, mind and body. - Coordinate with patients' existing primary care providers rather than duplicating their work. Qualifications - A Nurse Practitioner with Full Practice Authority in good standing (active U.S. licensure), OR a physician (MD/DO) board-certified in Geriatrics, Lifestyle Medicine, or Family Practice. - Multi-state licensure, or willingness to license in additional states, is a strong plus. - Trained or board-certified in lifestyle medicine (DipACLM / DipIBLM or active pursuit) — or deeply versed in its principles and eager to certify. - Experienced with, and genuinely energized by, caring for older adults. - A natural coach: warm, motivating, and skilled at helping people make changes that last. - Comfortable practicing via telehealth and excited by the pace and ambiguity of building something new. - Mission-driven, collaborative, and ready to be a key voice on our clinical team.
