Texas Nurse Practitioner (NP) – Part Time Remote Care Manager

Role Description We are seeking an experienced and empathetic Texas Nurse Practitioner (NP) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs—playing a vital part in our mission to enhance quality of life through proactive care management. Key Responsibilities - Conduct remote patient assessments with empathy and professionalism - Monitor patients' health status and escalate warning signs appropriately - Apply medical terminology knowledge to assess and coordinate care - Communicate clearly and compassionately with patients and families - Document thoroughly and accurately within multiple computer systems - Prioritize daily responsibilities while working independently - Provide excellence in patient care and customer service - Maintain patient confidentiality and HIPAA compliance at all times Qualifications - Current, active Texas NP license in good standing - Minimum 3 years of experience in a clinical or patient-facing setting - Minimum 3 years of experience in care management or case coordination - Prior experience working remotely preferred - Superior verbal communication and active listening skills - High proficiency in medical terminology - Excellent organizational and documentation skills - Professional phone etiquette and strong interpersonal abilities - Ability to recognize clinical red flags and escalate care as needed - Tech-savvy; able to work within multiple systems efficiently - Must have a HIPAA-compliant, private work area at home Benefits - 401(k) - Health insurance - Health savings account - Life insurance - Paid time off Job Type Part-time Application Question(s) - Do you have a minimum 3 years of experience in a clinical setting or patient-facing environment? - Do you have remote working experience? - Do you have a device that supports Windows 10 or newer, or a newer macOS version? - Do you have a current NP license in the state of Texas or have a compact license with Texas?
