Key areas you'll add value - Obtain provider and practice information through patient interviews and requesting their medical records directly from hospitals, physician practices, and other healthcare providers or locating them directly within the EMR systems of Diverge Health provider network partners. - Experience with organizing complete patient medical histories, including all primary care, specialist, behavioral health, labs, and visit notes from all relevant providers. - Complete assessments, tracking progress and attaching and uploading clinical documentation obtained from the above sources into the Diverge Health EMR, ensuring records are accurate, complete, and accessible for use by Diverge’s teams. - Transcribe and transfer clinical data from provider network partners EMRs into patient records within the Diverge Health’s EMR as necessary. - Follow-up with practices to resolve documentation gaps or outstanding records requests. Qualifications - Experience working in one or more EMR platforms, with the ability to navigate multiple systems efficiently. - Familiarity with clinical documentation, medical terminology, or healthcare operations. - Experience with, and a willingness to, interact frequently with healthcare administrators and physicians from outside of Diverge Health via telephone, email, and fax channels. - Experience with Salesforce or a comparable CRM or internal database system. - Strong attention to detail and a track record of maintaining accurate, organized records in a fast-paced environment. - Persistence in the face of administrative obstacles to data sharing. - Understanding of HIPAA regulations and knowing how to secure sensitive, protected health information (PHI) appropriately in various environments.
