Job Description

Integrated Clinic Pharmacist

CenterWell

Role Description We are seeking a Clinical Pharmacist with over 5 years of experience to join our primary care organization. You will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. The role requires strong critical thinking skills and the ability to use clinical judgment to make informed decisions about patient care. You will report to the Associate Director Integrated Clinic Pharmacist. This is a remote position. Candidates MUST be located in Florida and will work remote. Role Specific Responsibilities - Use evidence-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, including diabetes, hypertension, heart failure, and COPD. - Execute clinical protocols as allowed per organization and state regulatory agencies. - Optimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives, and home delivery pharmacy). - Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care. - Participate in high-risk rounds with a multi-disciplinary team to discuss patient progress and adjust treatment plans. - Excellent patient counseling skills and the ability to communicate complex medical information to patients and healthcare providers. - Disease state education for patients on appropriate use of medications and self-management. - Engage in quality improvement initiatives aimed at enhancing medication safety and efficacy. - Post-discharge medication reconciliation for high-risk transition of care patients. - Document all activity within clinic EMR and other required pharmacy or clinical management systems. - Support CMS Stars and HEDIS quality measures for successful clinic performance. - Maintain accurate documentation of clinical interventions and patient care activities. - Conduct in-service education and training sessions for healthcare staff and develop and share educational materials related to pharmacotherapy. - Stay current with advancements in pharmacotherapy and clinical guidelines. - Provide consultation to healthcare providers on complex medication management issues. - Respond to additional requests from the Primary Care Organization, providing support within the pharmacist's defined scope of practice to advance organizational goals. - Address and implement targeted initiatives or priorities as directed by the National Director or Associate Directors of Pharmacy, according to best practices and regulatory standards. Qualifications - Active and unrestricted Florida pharmacist license. - Willingness to become licensed in additional states for market support. - Pharm.D. or other advanced pharmacy degree. - 5+ years' experience as a practicing pharmacist. - Experience working in a team environment and supporting common goals. Requirements - Clinical residency in ambulatory care or similar discipline (preferred). - 2+ years working in an ambulatory care/primary practice setting (preferred). - Board certification in geriatric pharmacy or pharmacotherapy (preferred). - Familiarity with electronic medical record (EMR) systems (preferred). - Ability to identify and implement solutions for improved efficiencies and outcomes (preferred). - Multilingual (preferably English/Spanish) (preferred). Title: Encore Staff Pharmacist - Remote Location: Remote, US Admin/Corporate Services Job Description: POSITION SUMMARY The Staff Pharmacist interprets physicians’ prescriptions, contacts doctors or other prescribers and/or patients to verify information on prescriptions and expedite processing of order. This position is responsible for dispensing medications within corporate and regulatory guidelines. The Staff Pharmacist will track order status and outstanding issues as well as consult with patients regarding the use of medications and potential drug interactions. This individual may take calls from clients regarding orders or clarification, maintain daily production, quality and service levels, vary work priorities and activities to accommodate business needs. ESSENTIAL DUTIES - Interpret physicians’ or prescribers’ prescriptions. Contact doctors and/or patients to verify information on prescriptions such as drug strength, prescription sig., and drug name in order to expedite processing of orders. Verify and confirm validity of controlled substances. - Verify prescription information entered in the system by data entry or order entry. Contact physicians for new and/or transfer authorization.
