Licensed Physical Therapy Assistant, Care Navigator
Net Health
• Provide remote monitoring services for patients • Direct and assist users in therapeutic exercise and recovery • Provide ongoing feedback to the evaluating clinician • Maintain necessary records and notes while providing remote monitoring • Assist with improving processes and functionality of the platform • Organize and manage a caseload
Job Requirements
- 2+ years of experience in musculoskeletal health is a plus
- Graduated from an accredited physical therapy program
- Active PTA license in Illinois
- Tech savvy - thrive in a multi-platform environment
- Experience in behavioral modification; Health Coach Certification is a plus
- Excellent communication, interpersonal, and presentation skills
- Strong relationship-building and listening skills
- Demonstrated patient focus;
- Ability to multi-task and manage various types of users
- Ability to work independently with limited supervision
