Physical Therapist Assistant

Vori Health

• Provide physical therapy services by administering treatment protocols under the supervision of a physical therapist to support patients in their care journey. • Monitor patients' response to their plan of care and directly report any changes to the PT immediately. • Understand patient health by reviewing patients charts and studying therapy notes and histories. • Support patients in their virtual physical therapy program by instructing, encouraging, and assisting patients with their plan of care. • Increase effectiveness of Vori physical therapists by identifying patient care issues. • Documents patient care services by charting in patient records. • Documenting other clinical related activities as directed by your supervisor and Vori Health. • Maintains legal and accreditation compliance by adhering to policy positions concerning federal, state, and local regulations and JACHO standards. • Obtain and maintain multiple state licenses as directed by Vori Health (expenses related to licensure are paid by Vori Health). • Continue to update your knowledge of physical therapy through educational opportunities, reading periodicals, and professional contacts. • Enhances the patient experience at Vori Health through patient-centered virtual interactions. • Promotes the Vori Health reputation by commitment to high quality standards for physical therapy and exploring opportunities to add value to job accomplishments. • Communicate and assist other Vori care team members as needed to provide care and create an orderly and effective treatment of rehabilitation and physical therapy. • Meet company and quality standards as defined by Vori Health.

Job Requirements

  • Associate s degree for physical therapy assistant
  • Active, unencumbered PTA in your home state
  • Reside in a state which is part of the Physical Therapy Compact
  • Orthopedic Physical Therapist Assistant Expertise: At least 3 years of experience in digital care or outpatient physical therapy clinic evaluating and treating patients with a range of musculoskeletal pain conditions.
  • Demonstrated competence and passion for digital care delivery or telehealth: Comfort evaluating and treating patients remotely utilizing a variety of digital tools to manage care and experience administering a wide range of physical therapy techniques delivered digitally.
  • Proven record of matching patient needs to treatment plans and proven record of implementing these plans safely and effectively.
  • Strong experience administering a wide range of physical therapy techniques delivered digitally.
  • A strong sense of empathy and the ability to motivate patients into pushing their limits in a responsible, safe way.
  • Ability to create effective therapeutic alliances with patients while providing exceptional interpersonal and customer service skills.
  • Strong organizational skills and attention to detail while working across custom software during patient care delivery.
  • Excellent interpersonal, communication, organizational and problem-solving skills.
  • Critical thinking skills, flexibility, and the ability to design innovative solutions in a fast-paced environment.
  • Graceful at managing expectations while pushing for continuous improvement.
  • Comfort working within an environment of rapid pace and constant change.

Through Transitional Care Management and Triage, the RN educates and empowers women to manage their health conditions and achieve their individual goals of care. The RN role facilitates collaboration and continuity of care among team members, including providers, medical assistants, and health coaches. Essential Functions - Transitional Care Management (TCM): - Focused on outreach during the 30 days after discharge with the goal of reducing ER visits, hospitalizations, readmissions, and post-acute stays. - Reviews daily ADT feeds and initiates contact with patients within 2 days of discharge. - Schedule follow-up for TCM visit with the PCP within 7 days of discharge. - Complete weekly outreach for 30-day post-discharge. - Review discharge plan and ensures services (e.g. Physical Therapy, Home Health) are in place; provides community resources for SDOH (Social Determinants of Health) needs. - Provides patient/family education regarding health conditions and disease self-management. - Completes medication reconciliation and monitors adherence. - Documents all telephone/video interactions in EHR patient chart. - Identifies patterns and episodes of care that are predictive of future needs and services. - Coordinates care with other providers involved in the patient’s care as needed. - Triage: - Respond to patient telephone calls, refill requests, and electronic health messages. - Demonstrates critical thinking skills and expert nursing judgement to provide appropriate recommendations for care utilizing an approved evidence-based telephone nursing triage protocol. - Works with the RN scope of practice and standing orders to provide prescription refills, medication titration, and home treatments. - Educates patient regarding test results and medical conditions, and explains medical terms and procedures. - Maintains a professional demeanor and focus on patient-centered care. Qualifications - An active, unrestricted Minnesota RN license required, or ability to obtain by start date. Multi-state compact license preferred. - Willingness to obtain additional cross-state licensure as needed. - Minimum of 2 years’ experience as an RN in clinical setting. - Prior experience working as a triage RN or RN case manager required. - Exceptional communication skills and patient-centered focus. - Ability to monitor and evaluate opportunities for cost-effective care with high-quality outcomes. - Demonstrate effective, independent nursing judgement and skills. - Demonstrate skill and experience in effectively collaborating with care team members. - Demonstrate skill in effective use of electronic health records. - Demonstrate flexibility, organization, and appropriate decision-making in ambiguous situations.
