• Serve as Head of financial planning and analysis and run day-to-day operations of FP&A • Lead a team and scale as the business grows • Create monthly forecasts and build operating models, short-term and long-term financial plans • Be responsible for budgeting/planning processes and perform detailed department-specific budget and forecast reports • Partner with senior leadership to translate financial analysis into actionable business strategy and recommendations • Track key performance indicators (KPIs) and business metrics to drive accountability and performance • Oversee monthly, quarterly, and annual financial reporting, including variance analysis against budget and forecast • Lead the annual budgeting process and ongoing reforecasting in partnership with department leaders • Support fundraising, board reporting, and investor relations with financial models, analyses, and presentations • Establish and improve FP&A processes, systems, and internal controls to support a scaling organization
• Oversee the full-funnel demand generation and paid media strategy, owning CAC, lead volume, and lead quality targets across all paid channels. • Efficiently manage and deploy marketing dollars to deliver on new patient visit goals, shifting our channel mix from a lower-funnel-heavy model toward a more balanced push-pull engine. • Execute a rigorous test-and-learn approach across creative, audience, channel, and offer variables. • Build and manage campaign tracking and attribution infrastructure to establish true channel-level ROI visibility and enable data-driven optimization. • Own CAC, lead volume, lead quality, and funnel conversion metrics across all paid channels, with the ability to connect top-of-funnel performance to downstream patient conversion and retention outcomes. • Partner cross-functionally with Brand, Operations, IT, Sales, and clinical teams to ensure demand converts efficiently into new patients. • Contribute a data-driven POV to the broader growth leadership team, educating stakeholders on channel dynamics, attribution tradeoffs, and optimization priorities. • Manage agency and vendor relationships across paid media, direct mail, and marketing technology. • Serve as the internal subject matter expert on performance marketing, translating complex channel dynamics into clear strategic recommendations for senior leadership.

Within a value-based and patient-centered model of care reporting to the Company's Lead Registered Nurse, the Registered Nurse (RN) at Herself Health provides exceptional care to women ages 65 and up. Through Transitional Care Management and Triage, the RN educates and empowers women to manage their health conditions and achieve their individual goals of care. The RN role facilitates collaboration and continuity of care among team members, including providers, medical assistants, and health coaches. Essential Functions - Transitional Care Management (TCM): - Focused on outreach during the 30 days after discharge with the goal of reducing ER visits, hospitalizations, readmissions, and post-acute stays. - Reviews daily ADT feeds and initiates contact with patients within 2 days of discharge. - Schedule follow-up for TCM visit with the PCP within 7 days of discharge. - Complete weekly outreach for 30-day post-discharge. - Review discharge plan and ensures services (e.g. Physical Therapy, Home Health) are in place; provides community resources for SDOH (Social Determinants of Health) needs. - Provides patient/family education regarding health conditions and disease self-management. - Completes medication reconciliation and monitors adherence. - Documents all telephone/video interactions in EHR patient chart. - Identifies patterns and episodes of care that are predictive of future needs and services. - Coordinates care with other providers involved in the patient’s care as needed. - Triage: - Respond to patient telephone calls, refill requests, and electronic health messages. - Demonstrates critical thinking skills and expert nursing judgement to provide appropriate recommendations for care utilizing an approved evidence-based telephone nursing triage protocol. - Works with the RN scope of practice and standing orders to provide prescription refills, medication titration, and home treatments. - Educates patient regarding test results and medical conditions, and explains medical terms and procedures. - Maintains a professional demeanor and focus on patient-centered care. Qualifications - An active, unrestricted Minnesota RN license required, or ability to obtain by start date. Multi-state compact license preferred. - Willingness to obtain additional cross-state licensure as needed. - Minimum of 2 years’ experience as an RN in clinical setting. - Prior experience working as a triage RN or RN case manager required. - Exceptional communication skills and patient-centered focus. - Ability to monitor and evaluate opportunities for cost-effective care with high-quality outcomes. - Demonstrate effective, independent nursing judgement and skills. - Demonstrate skill and experience in effectively collaborating with care team members. - Demonstrate skill in effective use of electronic health records. - Demonstrate flexibility, organization, and appropriate decision-making in ambiguous situations. Nice-to-have Qualifications and Skills - Prior experience working in telephonic case management preferred. - Knowledge of Medicare and NCQA regulatory transitions of care preferred. - Experience in value-based care preferred. About the Clinic Therapist, LICSW: Reporting to the Company's Behavioral Health Care Manager, the Clinic Therapist, LICSW role will be a key part in creating a continued care plan for our patients through offering behavioral health services. Partnering with Providers, the Clinic Therapist, LICSW will help support behavioral health by providing basic counseling services. They will also address medication adherence barriers, connecting to community-based resources, and social determinants of health concerns by accurately documenting in our EMR (Athena). This role is expected to be a high-touch and visible role for our new and existing patients. This position is hybrid, splitting time between our Crystal and Rosedale Clinics, and remote (work from home - laptop provided). Clinic Locations: - Crystal Clinic | 5200 Douglas Dr N, Minneapolis, MN 55429 - Rosedale Clinic | 2401 Fairview Ave N #145, Roseville, MN 55113 Key Responsibilities: - Counseling and Psychosocial Support: Providing counseling and support to patients and their families to help them cope with illness, treatment, and life changes. This can include crisis intervention, grief counseling, and stress management. - Engages with patients and their families to provide telephonic support as a member of the interdisciplinary team. - Patient Advocacy and Support: Advocating for patients' needs and ensuring they receive appropriate and timely care. This includes helping patients navigate the healthcare system and accessing necessary resources. - Care Coordination: Collaborating with healthcare providers, patients, and families to coordinate care plans that address medical, social, and emotional needs. - Assessment and Evaluation: Conducting comprehensive assessments to identify patients' social, emotional, and environmental needs. This includes evaluating the impact of social determinants of health on patients' well-being. - Resource Connection: Connecting patients with community resources such as housing assistance, financial support, food programs, and mental health services. - Health Education and Promotion: Educating patients about their health conditions, treatment options, and healthy lifestyle choices. This can involve group education sessions or one-on-one counseling. - Cultural Competence: Ensuring that care is culturally sensitive and appropriate. Social workers must understand and respect the diverse backgrounds and experiences of their patients. - Outcome Monitoring and Reporting: Monitoring patient outcomes to ensure that care plans are effective and making adjustments as needed. This includes documenting and reporting on patient progress and outcomes. - Policy Advocacy: Advocating for policies that improve access to care and address social determinants of health.
