Role Description Chronic Care Staffing is seeking an experienced and compassionate Remote Care Coordinator (RN) to join our chronic care management team. This fully remote position is ideal for a registered nurse who thrives in a structured telehealth environment and is passionate about improving outcomes for patients managing chronic conditions. In this role, you will provide care coordination, patient education, and remote monitoring support while serving as a key communication bridge between patients and providers. Your work will directly support quality care delivery and long-term patient success.
