Responsibilities - Provide remote monitoring services to help support patients as they progress through their home-exercise therapy program. - Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform. - Provide ongoing feedback to the evaluating clinician on patient progress outside of the clinic. - Maintain necessary records and notes while providing remote monitoring. - Assist with improving processes and functionality of the platform. - Organize and manage a caseload while staying efficient. Qualifications - As a PTA, you are naturally empathetic, friendly, and love what you do! - 2+ years of experience in musculoskeletal health is a plus. - Graduated from an accredited physical therapy program. - Active PTA license in a compact state. - Tech Savvy - thrive in a multi-platform environment, comfortably switching between systems (EHR, messaging, scheduling) without missing a beat. - Experience in behavioral modification; Health Coach Certification is a plus. - Excellent communication skills, interpersonal, and presentation skills, and an ability to collect and present meaningful data to drive decisions. - Strong relationship-building and listening skills. - Demonstrated patient focus; a genuine passion for the user and their experience. - Ability to multi-task and manage various types of users. - Ability to work independently with limited supervision. - The flexibility and agility to work cross-functionally in a fast-paced environment. - The ability to influence change and bring enthusiasm and excitement to the workplace. - Independent and proactive work style; quick, flexible thinking; and an open, inquisitive approach. - Eager to make an impact at a young company addressing an important problem. - Passion for health and wellness. Role Description This is a telephonic, queue-based position supporting outpatient services for children and adolescents. As a Behavioral Health Advocate, you will respond to treatment requests from providers and facilities, as well as manage and deescalate crisis calls from members. Primary Responsibilities: - Work with providers, facilities, and members to perform utilization management and authorization for outpatient services. - Ensure that behavioral health care services provided are medically necessary, per standard guidelines. - Ensure that behavioral health care services are provided in the most appropriate setting. - Integrate a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services to meet the enrollee’s health needs, using education, communication, and all available resources to promote quality, cost-effective outcomes. - Discharge planning or management of transitions between care settings to ensure the appropriate services/resources are in place for quality outcomes. - Refer members to appropriate psychiatric and community referrals and assist with identifying and removing potential barriers. Qualifications - One of the following licensures - must be active and unrestricted: - Independently licensed master’s degree in psychology, social work, counseling or related field (i.e., LPC, LPCC, LCSW, LISCW, LMFT, LMHC, etc.) - Licensed PhD/PsyD (LP) - Registered Psychiatric Nurse (RN) - 3+ years of post-licensure clinical experience in a related behavioral health environment. - Proficient with MS Office Suite. - Access to install secure, high-speed internet and a dedicated, distraction-free workspace at home. - Available to work one weekday holiday per calendar year. - Desire to work in a telephonic, queue-based position responding to treatment requests and crisis calls. - Experience working with children, adolescents, and families. Role Description The Telemedicine Intensivist Advanced Practitioner Provider (APP) is a certified, licensed nurse practitioner or physician assistant who independently and collaboratively works with the multidisciplinary team to provide comprehensive care to acutely ill and/or medically complex patients in the intensive care unit via telemedicine.
