Role Description - Participate in telemedicine outreach by placing outbound phone calls to patients and providers to facilitate linkage to medical, behavioral health, and social services. - Assist Health Navigators in connecting patients to follow-up care for communicable diseases, STIs, and SUDs. - Document patient interactions, referrals, and outcomes in the electronic medical record (EMR) and related tracking tools. - Collect, log, and maintain patient data in Excel and Google Sheets with accuracy and confidentiality. - Handle protected health information (PHI) in strict accordance with HIPAA and organizational privacy standards. - Provide general administrative and operational support for day-to-day business transactions. - Attend team meetings, case conferences, and cross-functional discussions; contribute observations and insights where appropriate. - Collaborate with the Practice Lead, Health Navigators, and clinical staff on active projects, offering assistance and fresh perspectives. - Support special projects and pilot initiatives as assigned, including data collection, research, and process-improvement efforts. - Provide support for general business transactions. - Attend meetings and provide insight into discussions. - Collaborate on specific projects, offering assistance and contributing fresh ideas. - Participate in special projects as needed. - Attend training sessions or workshops to enhance skills and knowledge. - Seek feedback and actively engage in the learning process. Responsibilities Clinical Responsibilities: - Complete initial diagnostic assessments on all patients assigned to determine - treatment needs, utilizing the assessment data to support a DSM V preliminary diagnosis. - Refer patients or family members to community resources or specialists when a need is identified in the assessment process. - Educate patients about crisis planning, and how to access crisis services, including the 988 Suicide & Crisis Lifeline. - Maintains effective and timely documentation. - Formulates appropriate treatment plan along with consumer and families, as appropriate. - Provides individual, group and family therapy, utilizing basic therapeutic models and theories according to consumer needs as stated in the treatment plan. - Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation. - Effectively collaborates with the treatment team members and outside sources in the delivery of clinical services. - Provides support and education for consumers and family members. - Evaluates the consumer for discharge planning. - Facilitates services that prevent commitment. - Adheres to professional code of ethics. - Effectively demonstrates the application of adult and child/adolescent specific competencies. - Assists with seclusion and restraint per AltaPointe policy and procedures. Qualifications Licensure required: LPC, LMFT or LCSW Master’s degree in a mental health discipline; 23 years of age; valid driver’s license; successful completion of a practicum as part of the requirements for the degree or has six (6) months of post master’s level clinical experience supervised by a master’s level clinician with two years of postgraduate clinical experience.
