Responsibilities - Évaluer le fonctionnement occupationnel ainsi que les capacités fonctionnelles, cognitives et psychosociales des client·e·s. - Identifier les obstacles à la participation dans les différentes sphères de vie (travail/études, routines, relations sociales, loisirs) et au retour au travail. - Élaborer et mettre en œuvre des plans d’intervention et de traitement individualisés, fondés sur des données probantes, alignés sur les forces, les besoins et les objectifs de la personne. - Intervenir sur la gestion du stress, de l’anxiété, de l’humeur, de la motivation, de l’organisation, des habiletés sociales et des stratégies d’adaptation/autorégulation. - Soutenir la réadaptation en santé physique (p. ex. blessures musculosquelettiques/orthopédiques), la sécurité à domicile, l’ergonomie et les programmes de retour/maintien au travail. - Accompagner des clientèles en invalidité de longue durée (LTD) et des vétérans dans un parcours gradué et durable de réintégration occupationnelle. - Collaborer au besoin avec une équipe interdisciplinaire (psychologues, travailleurs sociaux, médecins, physiothérapeutes, employeurs, assureurs, SAAQ/CNESST, etc.) pour coordonner des soins intégrés et obtenir des résultats optimaux. - Éduquer les client·e·s et leurs proches sur les stratégies de rétablissement, de prévention et d’autogestion. - Consigner l’évaluation, les interventions et les recommandations cliniques (notes évolutives, rapports) dans le dossier électronique du client. - Utiliser des outils modernes (dossiers électroniques, communication numérique) pour simplifier le flux de travail. - Participer à l’apprentissage continu et au perfectionnement professionnel; mentorer ou être mentoré·e au sein d’un réseau de professionnels de qualité. - Faire preuve d’un jugement clinique solide, d’une approche humaine et d’un fort esprit d’équipe.
