The Outpatient Clinician – Telehealth is part of a Virtual Hub team that provides clinical services for individuals and families that present for telehealth treatment. The Virtual Hub is part of an integrated model of care and is complemented by physical office locations with services that can be offered in-person, in the community and/or via telehealth to provide a holistic healthcare experience. In collaboration with a treatment team, the Outpatient Clinician – Telehealth provides services including but not limited to treatment planning and on-going assessment; client education; crisis intervention and risk assessment; individual, family, group and collateral therapy, referral and advocacy. Caseloads include diverse populations with a variety of clinical needs. The Outpatient Clinician – Telehealth is a positive team player that promotes a culture of inclusion and participation. Required Education and Experience - Master’s degree in Marriage and Family Therapy, Professional Counseling, Psychology, or Social Work - Licensed Independent Practitioner by Kentucky professional boards and billing sources (e.g. LCSW, LPCC, LPP, LMFT, PhD clinical psychologist, etc.) - Meet the requirements of applicable Kentucky licensure boards to provide services to individuals and families residing in the state of Kentucky Preferred Education and Experience - Experience working in a behavioral health field Additional Eligibility Qualifications Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Clinical Services - Work as part of a team to deliver high quality therapeutic care from a whole health perspective - Deliver evidenced based counseling that provides effective medically necessary treatment of mental and behavioral conditions in individuals and families - Assess and integrate historical, psychosocial and medical information into a client’s individualized treatment - Monitor the need for continuation of services and/or change in level of intensity of services - Utilize PCOMS (Partners for Change Outcome Management System) in assessments and clinical treatment episodes - Deliver services primarily in outpatient home-office setting - Deliver services via telehealth and coordinate with the treatment team to ensure client needs are met in person or via telehealth - Manage situations involving client risk and provide clinical crisis services to de-escalate crisis situations and promote stability - Manage a caseload that may include diverse populations with a variety of clinical needs - Maintain a professional and secure home-office environment - Engage treatment team, New Vista specialty programs and community partners to advance relationships to provide whole health care to address behavioral health, physical health and social determinant needs Collaboration and Communication - Collaborate with client, family (if appropriate) and treatment team to develop treatment goals - Communicate effectively with team members and collateral sources - Refer and transfer when clinically indicated - Participate in regular meetings with Clinic Manager and treatment team - Maintain an integrated care approach by engaging client’s local resources including care team (internal and external to New Vista), family, friends, school, and other community resources, as appropriate Documentation - Manage medical records for clients receiving services - Secure authorizations for medically necessary clinical services - Document to meet medical necessity standards - Submit documentation within required timeframes
