As an Account Manager, you will support patients transitioning from hospital to home by coordinating specialty and home infusion services. You will partner with healthcare providers, home health agencies, and internal teams to ensure seamless access to therapy, clear communication, and positive patient outcomes. This role blends patient advocacy, coordination, and relationship management to deliver high‑quality, cost‑effective care. Responsibilities - Coordinate the continuum of care for patients receiving Specialty IV Therapy, Home IV Therapy, Specialty Pharmacy, or Enteral Nutrition services. - Collaborate with hospitals, clinics, physicians, and home health agencies to ensure timely and effective home‑based care. - Manage the full referral lifecycle, including insurance verification, home health scheduling, and coordination with operational teams. - Educate patients and caregivers on therapy expectations and financial responsibilities with empathy and clarity. - Build and maintain strong relationships with referral sources while supporting growth across assigned hospitals and clinics. - Identify patient and family concerns and assist with issue resolution to promote satisfaction and continuity of care. - Maintain accurate documentation and ensure patient confidentiality at all times. - Participate in required meetings, in‑services, and training activities. - Comply with established administrative, HR, and organizational policies. Required Qualifications - Strong customer focus with the ability to identify and respond to patient and referral source needs. - Excellent communication and interpersonal skills. - Strong organizational, analytical, and problem‑solving abilities. - Ability to manage multiple priorities and work independently in a fast‑paced environment. - Proficiency in Microsoft Word and Excel. Preferred Qualifications - Bachelor’s degree or equivalent experience. - Experience in managed care, insurance, or healthcare coordination. - Project management or public‑speaking experience. - Ability to collaborate with diverse stakeholders across healthcare settings.
