Purpose: This position works with a professional team of physicians and other staff who are trained in the medical and surgical specialty of dermatology with the purpose to be patient focused with the objective of delivering superior patient care and ensuring a positive patient experience and outcome. The position serves patients and visitors by answering phones, assisting patients with registration and intake, scheduling appointments and taking payments over the phone and in person. This position is supervised by the Lead, Customer Service and/or Manager, Customer Service. Essential Duties and Responsibilities: - Serves as first point of contact for incoming billing customer service calls. - Provides optimum customer service, including quality assurance, for all patient inquiries. - Answer high-volume incoming communications from customers/patients. - Returns all phone messages within 24 hours of receipt. - Must be able to deal diplomatically with complaints and function well under pressure. - Utilize resources and conduct research to troubleshoot and resolve patient issues. - Reviews patient account data when interacting with patients to ensure that accurate information regarding patients’ insurance and demographic information are correctly reflected on the patient’s account. - Updates insurance and demographic information as necessary on patient accounts. - Accurately documents patient accounts of all actions taken. - Set up payment plans and take credit card payments over the phone. - Effectively uses billing system to document patient accounts using proper grammar, spelling, and syntax. - Participates in special projects as needed. - Maintains regular and predictable attendance. - Adheres to Code of Conduct and Mission & Value Statement. - In addition to the above job responsibilities, other duties maybe assigned.
Job Requirements
- High School graduate or equivalent.
- Knowledge of health plan benefits (deductibles, copays, coinsurance) and eligibility verification.
- Must be able to read, interpret and explain EOBs and patient responsibility in detail.
- Education in medical billing or hold medical billing certification preferred.
- Mod Med/EMA practice management system experience is a plus.

Role Description Form Health is currently seeking an experienced Registered Dietitian Diabetes Specialist to join our cardiometabolic team. The Registered Dietitian will be responsible for providing individualized, ongoing, comprehensive medical nutrition therapy to patients through our telehealth platform. The ideal candidate has a strong background in the field of diabetes and weight management, a positive, non-judgemental attitude, takes initiative, and is looking to continuously grow and improve our cardiometabolic program. This is a full-time, 40-hour-per-week position with flexible schedule options. Job Responsibilities: - Work closely with Form Health obesity medicine physicians and endocrinologists, PAs and NPs to provide intensive, ongoing medical nutrition therapy support to patients via regular video visits. - Establish a professional and effective relationship with all patients, providing care that is comprehensive, empathetic, and evidence-based. - Create customized nutrition plans for patients based on their individual needs. - Provide nutrition education and counseling around diabetes self-management (primarily Type 2 Diabetes), healthy eating, and behavior change. - Act as a clinical specialist in diabetes including data interpretation with the use of Continuous Glucose Monitors (CGMs) or glucometers. - Support patients with complex co-morbidities including Chronic Kidney Disease (CKD) and Cardiovascular conditions. - Review and offer feedback on patients’ daily food photos. - Support patients in between visits via daily text messaging in the Form Health app. - Work to grow and enhance our cardiometabolic program in a fast-paced, ever-changing environment. Qualifications - Registered Dietitian with at least 3-5 years experience in counseling, behavior modification techniques, and working with patients with diabetes (primarily Type 2 Diabetes). - Certified Diabetes Care and Education Specialist (CDCES) highly preferred. - Experience working in medical weight loss, with patients on weight loss medication, using meal replacement plans, leading support groups, and strong cognitive-behavioral therapy counseling techniques.
