Role Description We are seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity, and to collaborate with advanced practice professionals and registered dietitians towards exceptional team-based care. Work is remote, and schedules can be flexible. We are currently accepting applications for a FTE (40 hours per week) Obesity Medicine Physician or Endocrinologist opportunity, who are licensed in GA. This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage. This is a Modified Full Time position based in a remote, telemedicine clinical setting. Qualifications - MD or DO degree with board certification; must be an American Board of Obesity Medicine (ABOM) diplomate or board certified in Endocrinology. - Licensed to practice and prescribe legally in GA. - LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) is preferred. - Credentialed with insurance payers (strong preference for multiple state licenses). - Excellent interpersonal skills for patient and team interactions. - Committed to the highest quality of medical care and an evidence-based approach to obesity management. - Prior experience conducting telemedicine visits with a great "web-side" manner. - Prior experience working collaboratively with advanced practice professionals and registered dietitians. - Bilingual candidates with medical fluency in Spanish are highly preferred. Role Description We are seeking a Business Operations Associate who will serve as a strategic partner across the business, helping to sharpen data-driven decision-making and help scale our analytical capabilities as we grow. About the Role: We are hiring experienced Nurse Practitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity. In this role, you would collaborate with physicians and registered dietitians to deliver exceptional team-based care. This will be a full-time, remote position. We seek applicants who: - Have a Nurse Practitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy - Are licensed to practice and prescribe legally, with a preference for credentialing with major insurance payers, in the state of California - Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care, internal medicine, or similar setting - Have excellent interpersonal skills for patient and team interactions - Are committed to the highest quality of medical care and an evidence-based approach to obesity management - Have prior experience conducting telemedicine visits and who have a great "web-side" manner - Bilingual candidates with medical fluency in Spanish are highly preferred More about Form Health’s benefits: - Competitive salary in a high growth start-up - Comprehensive health benefits that start day one - 401k program - Flexible work schedules and paid time off - Paid parental leave - Ability to participate in Form4Form, Form Health's weight loss program available to all employees and their dependents.
