About our Centralized Follow Up Specialist position JOB SUMMARY: The Centralized Follow Up Assistant is a non-licensed member of the clinical and administrative Centralized Follow Up [CFU] team that provides patient follow up management and affiliate health services in accordance with Planned Parenthood policies, protocols, and procedures. What will you be doing! - GENERAL: Works in accordance with affiliate Medical Standards & Guidelines [MS&Gs]; provides education and advice to patients within affiliate protocols and scope of practice as defined below. - TEST RESULTS: When patients reach out to CFU to request their lab results, the CFU Assistant can notify patients of normal test results (with the exception of Cervical Cancer Screening [CCS] tests and imaging results) and document that notification accordingly. The CFU Assistant will task the licensed CFU team members to review normal CCS results, normal imaging results, and all abnormal results to connect with the patient to review those results - NOTIFICATIONS AND REMINDERS: Send 3rd notification letters and reminder letters for recommended treatment/follow up/referrals and document their completion. - SCHEDULE APPOINTMENTS: Schedule appointments for patients for specific treatment, follow up of abnormal lab results, or in-house referrals and document their completion. - CONFIDENTIAL MORBIDITY REPORTS [CMRs]: Complete and transmit CMRs for all positive reportable communicable diseases (chlamydia, gonorrhea, syphilis, herpes, HIV) to the county and/or state Public Health Department. Document CMRs accordingly. - EXPEDITED PARTNER THERAPY [EPT]: Transmit or phone in EPT to preferred pharmacies for patients’ partners in chlamydia, gonorrhea, and trichomoniasis cases, according to state rules and regulations. Document EPT prescriptions accordingly. - MAILING DOCUMENTS TO PATIENTS: Print, pack, stamp, and mail hard copy letters via USPS in a HIPAA-compliant manner. - AFTER HOURS CONSULTING NURSE COMMUNICATIONS: Scan in all after hours consulting nurse faxes into the Electronic Health Record [EHR] under the correct patient and task the appropriate CFU Triage RN for review and care coordination, as needed. - MY CHART MESSAGES: Respond to MyChart messages that request non-clinical needs (ie, work excuse note, medical records requests) or normal lab results (see above). - PRIOR AUTHORIZATIONS: Review, process, and notify patients of Prior Authorization claims for medications and procedures. - MEDICAL RECORDS: Abstract incoming medical records and task them to CFU Advance Practice Clinicians [APCs] to review; respond to incoming Release of Information [ROI] requests; forward ROI requested by subpoena to the Quality department for legal review. Document receipt or release of medical records accordingly. - HEDIS: Review and respond to HEDIS requests from payors. - OTHER DUTIES: Performs other duties as assigned; nothing in this job description restricts management’s right to assign or reassign duties and responsibilities to this job at any time. Internal Responsibilities: - Utilize client CDI technology and follow established standardized process flow - Maintain open communication with coding to discuss DRG assignment, diagnosis, clinical indicators, coding clinics and guidelines and educating each other on specialty. - Assign Working DRG for Case Management department to view Length of Stay (LOS) of patients. - Participate and provide input regarding CDI program activities, and attendance at routine team meetings. - Maintain current skill set with regard to government regulations, compliance and reimbursement guidelines. - Expected to keep abreast of new legislation and regulations that affect CDI. - Maintain personal and professional education and growth. - Responsible for maintaining continuing education credits as required by credentialing organization.
