Job Description: Nurse Practitioner / Physician Assistant Monogram Health is looking for skilled Nurse Practitioners and Physician Assistants eager for the opportunity to make a difference in patients' lives. Your Impact Using your skills in this position will allow you to deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD. You will also be responsible for caring for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments to patients as required. The company will support your certification completion through onboarding. - Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license - Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA) - Current and unrestricted DEA certificate - Ability to work without direct supervision and practice autonomously - Access to transportation, a valid driver's license, and car insurance - Must be proficient with medical instruments and equipment required by the work - Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology - Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians and representatives of the community  - Sound understanding of all federal and state regulations including HIPAA and OSHA - 2 or more years of direct patient care required - Managed Care/IPA/Health Plan experience - Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home  About Monogram Health Monogram Health is a leading multispecialty provider of in-home, evidence-based care for the most complex of patients who have multiple chronic conditions. • Provide high-quality synchronous instruction for online, masters-level Marriage and Family Therapy courses • Infuse integrative systemic theory and practice into course material • Facilitate synchronous sessions with 15 or fewer students each week in a regularly scheduled 90 minute class • Grade and return all papers and assignments within two weeks or earlier • Meet occasionally with the faculty Course Lead to discuss course procedures, program updates, curriculum development, and issues specific to students in your course section • Provide additional support and guidance to students as needed

• Provide high-quality synchronous supervision for online, masters-level Marriage and Family Therapy Internship courses • Infuse integrative systemic theory and practice into course material • Monitor the students’ accomplishment of clinical training requirements • Communicate, Coordinate, and Consult with Student’s Site Supervisor regarding student training and performance • Submit grades to faculty Course Leads in a timely fashion each quarter

What will you do? - Conduct intensive nutrition counseling and weight management education via phone, video, chat, text, and email. - Provide medication management and support for comorbidities related to obesity under the supervision of assigned physicians. - Engage in remote patient monitoring for various devices (e.g., scale, BG meter, BP monitor, ketone meter). - Support patients with dosing and titration of anti-obesity medications within established protocols. - Collaborate with team physicians and external providers to ensure comprehensive care. - Quickly establish rapport and therapeutic alliances with a diverse patient population. - Utilize motivational interviewing and other techniques to support behavior change. - Consistently meet program metrics while maintaining high-quality care standards. - Complete all necessary documentation in a timely manner. What do you need to bring to the table? - Must be a Registered Dietitian credentialed by the Commission on Dietetic Registration. If required, you must hold state licensure. - Willingness and ability to add state licenses as needed to support member coverage. - Able to incorporate constructive feedback to improve patient interactions. - Must have a HIPAA friendly workspace with high-speed internet for secure patient conversations. - Comfortable with advanced technology, including electronic medical records and various digital tools. - Demonstrated ability to adapt communication techniques to meet the needs of a diverse patient population. - Flexible schedule with a commitment to a minimum of 20 hours per week. - Cannot hold full-time employment elsewhere. - Must live in the United States. What are some “nice to haves”? - Certification from the National Certification Board for Diabetes Educators (NCBDE) as a Certified Diabetes Care and Education Specialist (CDCES) is strongly preferred. - Availability on evenings and Saturdays. Compensation: - Pay: $42-47 per hour based on years credentialed by the CDR as a Registered Dietitian. - All-inclusive pay structure: hourly compensation for all work performed - including charting and administrative time (ie: not a flat fee per session). - Longevity recognition: Receive a $500 bonus after 6 months, followed by quarterly bonuses beginning at your 1-year anniversary. - Performance-based incentives: Earn additional pay for achieving and exceeding goals.
