• Collaborates with members of an inter-disciplinary care team (ICT), providers, community and faith-based organizations to improve quality and meet the needs of the individual • Facilitate communication, coordinates care and service of the member through assessments, identification, person-centered planning • Assist the member in creation and evaluation of person-centered care plans • Engage the member and their natural support system through strength-based assessments using a trauma-informed care approach • Develop and regularly update a person-centered individualized care plan (ICP) in collaboration with the ICT • Monitor and promote effective utilization of healthcare resources through clinical variance and benefits management • Document care coordination activities and member response in a timely manner according to standards of practice and CareSource policies

• Oversee the full lifecycle of care coordination for members in the Virtual Intensive Outpatient Program (VIOP) • Screening and qualifying prospective members for program eligibility • Conduct clinical outreach to families and onboarding new members • Manage case coordination and facilitate communication across multidisciplinary teams • Support members and caregivers throughout their VIOP journey • Ensure timely scheduling of intake assessments once members are qualified for VIOP enrollment • Orient members and their families to the VIOP program • Serve as the primary point of contact for members and families • Coordinate external case management needs with schools, Primary Care Providers, and community resources • Assist with discharge planning and transition to lower levels of care or additional services • Maintain accurate and timely documentation in the electronic health record (EHR) • Act as a liaison between families and the VIOP treatment team • Collaborate with internal teams to address barriers to care • Conduct comprehensive biopsychosocial assessments for members entering the VIOP • Develop individualized care plans in collaboration with the treatment team • Support group facilitators during sessions • Provide clinical session coverage as needed

• Responsible for providing day to day leadership, control over project schedule and risks, scope of work, and mentorship to your team(s) • Review, propose, design hybrid and Multicloud network solutions to meet existing, and proposed technological and business outcomes of your client • Ensure team members follow established procedures and deliver exceptional client service across all aspects of delivery management • Engage with clients to exchange information, gather requirements and expectations and ensure high levels of satisfaction • Monitor team performance and outcomes, addressing and resolving competency and related issues • Build and maintain strong client relationships and provide day-to-day client advice and support • Manage the delivery of contracted services to clients to ensure time, quality and cost of delivery

• Assist in the development of strong and captivating proposals to meet client needs, competitiveness, and cost-effectiveness based on short research briefs and/or elaborate Request For Proposal, including assisting with costing structure, methodological design, analytical framework, etc. • Write the MoM (Minute of Meetings) of kick-off or any meetings with clients. • Prepare screeners for recruitment. • Deliver fieldwork briefing to ensure the interviewers are clear about the purposes. • Collaborate with the Project Manager to ensure the progress of recruitment is well communicated and monitored. • Design discussion guides to ensure they prompt meaningful findings and are actionable. • Provide moderation for basic qualitative methodologies, such as focus groups and in-depth interviews. • Convert the interview's verbatim to Content Grid and create the Content Analysis of the interview. • Consult with Research Manager (Questionnaire Design, Analysis, Reporting, & Writing). • Manage multiple projects simultaneously.
