Job Description

Triage Nurse, Telehealth

Guideway Care

• Conduct thorough clinical assessments and triage patients based on established evidence-based protocols and clinical guidelines • Respond to patient inquiries via inbound and outbound calls, as well as secure patient messaging portals • Provide appropriate recommendations for care or escalate concerns to the appropriate provider or care team member • Document assessments, communications, and care coordination activities in electronic medical records (EMRs) with accuracy and clarity • Support administrative functions related to patient care including FMLA, ADA, STD/LTD documentation, and coordination • Support training and onboarding of team members, including participating in shadowing shifts (both as a learner and trainer), while reinforcing best practices to ensure consistent, high-quality patient care • Collaborate with healthcare providers, clinical teams, and practice administrators to ensure continuity and quality of care • Uphold Sequence Health’s commitment to clinical excellence, integrity, and service in every interaction

Job Requirements

  • Active, unencumbered Registered Nurse license in a compact licensure state
  • Minimum of 5 years’ experience in working in a variety of direct patient care settings including Emergency Department, Labor and Delivery, Critical Care
  • Women's Health or Labor and Delivery experience preferred
  • Prior remote RN triage experience or experience with telephone-based care delivery strongly preferred
  • Demonstrated clinical proficiency with strong critical thinking, problem-solving, and decision-making skills
  • Excellent verbal and written communication skills, with the ability to convey empathy and clinical clarity via phone and written messages
  • Proficiency in using EMRs (e.g., Epic, Cerner, Greenway) and standard office software (Microsoft Office, Adobe, fax, etc.)
  • Ability to work independently within scope, while maintaining alignment with clinical protocols and best practices
