• Provide quality customer service by obtaining necessary documentation to underwrite the loan • Communicate directly with the customer and/or Loan Officer • Verify all required documentation (pre-underwrite) in the file • Prepare a loan approval summary, Forms 1008 and 1003 • Communicate to obtain initial and any missing documentation required to qualify for the loan • Review loans for compliance with all mortgage-related regulations • Document all communication related to the loan application in the conversation log • Train and assist supervision of new Loan Processors and other processing personnel

• Assist in reviewing mortgage loan application and documentation • Review mortgage loan disclosure packages and deliver them to the borrower • Create rate disclosure and re-disclosure packages and deliver them to the borrower • Review documents and entire loan files • Clarify/correct information and obtain additional information • General clerical and phone support to loan officers and/or loan processors • Work with management to provide support in various stages and areas of lending and processing • Deliver appraisals and/or property valuations to the borrower • Track outstanding items on all loan files • Facilitate timely loan closings by working directly with the Title Company and customer to schedule

- Utilize our specialized care management software to call Medicare patients with 2 or more chronic conditions (Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis - Build and maintain rapport with patients to help coach them to improved health through SMART goals and education on self-management strategies - Implement and improve the Plan of Care by updating medications, appointments due, biometrics, symptoms, and interventions made - Connect the patient with community resources as needed, including transportation, personal care needs, prescription/DME assistance, social services, etc. - Conduct Transitional Care Management activities to high-risk patients discharged from the hospital and the ER to reduce unnecessary readmissions. - Close care gaps by encouraging and assisting with preventive care measures, i.e. annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.

• This position requires the ability to have a compelling conversation and to provide high quality customer service for donors on the NMDP registry who are being asked to save a patient’s life. • The role involves using investigative skills in partnership with information from within NMDP systems to locate and contact donors. • Candidates must effectively engage donors who could have misconceptions about the donation process. • Responsibilities of this role include the ability to access information from within NMDP systems, confirm interest, and assess health conditions, update donor demographics, and complete/manage all other related record updates and documentation accordingly. • The following activities would also be performed as necessary: scheduling laboratory testing, ordering/sending donor kits, obtaining consent, and monitoring donor follow through. • Good guardianship of member confidentiality and identity is required. • Role supports the achievement of department goals through job accountabilities, excellent customer service, and exceptional time management.
