The Role We are hiring Care Coordinators (1099 Contractors) to provide Chronic Care Management (CCM) services to patients with chronic conditions. This is a phone-based, productivity-driven role where you will spend most of your workday on the phone speaking with patients, coordinating care, and documenting interactions. This role is ideal for clinical professionals who are comfortable with high call volume and meeting performance metrics in a remote environment.
