Responsibilities Key Responsibilities: - Conduct comprehensive assessments of students' speech, language, and communication abilities. - Develop individualized therapy plans and goals based on assessment results and students' needs. - Provide direct therapy services to students, targeting areas such as articulation, phonology, language comprehension, fluency, and pragmatic skills. - Collaborate with teachers, parents, and other members of the educational team to support students' communication goals in the classroom and other educational settings. - Incorporate evidence-based techniques and therapeutic activities into therapy sessions to promote speech and language development. - Monitor students' progress and adjust therapy plans as needed to facilitate optimal outcomes. - Document therapy sessions, progress reports, and other relevant information accurately and in a timely manner. - Participate in IEP (Individualized Education Program) meetings and provide input regarding students' speech and language needs. Qualifications Qualifications: - Master's degree in Speech-Language Pathology from an accredited program. - Valid state licensure or eligibility for licensure as a Speech-Language Pathologist. - Experience providing speech-language therapy services in a school or pediatric setting. - Proficiency in assessment tools and therapeutic techniques commonly used in pediatric speech-language pathology. - Excellent communication and interpersonal skills, with the ability to collaborate effectively with students, parents, and school staff. - Strong problem-solving abilities and attention to detail. - Experience in teletherapy or online counseling (preferred). - Knowledge of augmentative and alternative communication (AAC) systems and other assistive technology used in speech-language therapy (preferred). We are a "say yes" first clinical provider and look to treat patients where they are and guide them along their journey to recovery and stabilization. *Clinician must have an active, unrestricted behavioral health license in California Key Areas of Ownership: - Provides patient care modalities of evidence-based practices to best meet the clinical needs of the patients. Examples include; CBT, DBT, Solution-Focused Therapy, Psychoeducation (Symptom Recognition), Trauma Therapy, Addiction Therapy, Acceptance and Commitment Therapy, Bereavement/Grief Counseling, etc. - These specific treatment modalities will be implemented in both the individual and group therapy setting, with a focus of engaging patients in their treatment plan. - Review and signature of patient clinical Psychosocial assessment, diagnosis, treatment plan, and consents as needed for the patient in a timely manner. Maintenance of the clinic record, including documentation of activities performed as part of the service delivery process. - Documents all services and patient activities appropriately. Ensures documentation complies with best clinical practices, contracts, regulatory and other legal - Conducts crisis intervention for the individual and family based on the individual’s safety plan. - Participate in regular case planning with the involved Multidisciplinary Team as new information is gathered to ensure the most clinically appropriate patient disposition. - Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Role Description The Patient Outreach Supervisor will be a front-line manager for the Patient Outreach team, a team dedicated to communicating with potential patients about the value of hospital base pharmacies. They will be responsible for the development, maintenance, and growth for a team of highly efficient and trained Patient Outreach Coordinators and Onboarding Specialists. They will also maintain and build strong collaborative relationships with their partnering Hospital sites and regional operations teams. The Patient Outreach Supervisor will counsel Patient Outreach Coordinators and Onboarding Specialists on incorrect conversational topics, documentation or workflow deficiencies, and will escalate other performance concerns to the Patient Outreach Development Manager.
