Location: Remote (Work from Anywhere in the U.S.) Pay Range: $17.00 – $20.00 per hour Employment Type: Full-Time Overview Chronic Care Staffing is seeking a detail-oriented and patient-focused Remote CCM/RPM Enrollment Coordinator to support our chronic care management programs. This role is ideal for a Certified Medical Assistant (CMA/RMA) who enjoys engaging patients, explaining healthcare programs, and helping individuals actively participate in managing their chronic conditions. In this position, you will introduce patients to Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services, obtain verbal consent, and serve as a trusted liaison between patients and care teams.
