Patient Care Support - Patient Liaison/Triage: Serve as the primary non-clinical contact for patients, handling routine inquiries, and efficiently triaging urgent medical or clinical needs to the Registered Nurse (RN). - Referral Management: Coordinate, track, and follow up on referrals to external specialists, counseling services, and higher levels of care to ensure seamless continuity. - Intake Preparation: Proactively support new patients by completing necessary intake paperwork and preparation ahead of their initial clinical appointment. - RN Support: Actively manage patient requests and follow up on tasks delegated by the RN team, prioritizing clinical flow efficiency. - Documentation: Accurately enter and update patient demographics, medical history, lab results, and communication logs in the Electronic Health Record (EHR). Pharmacy & Medication Logistics - Pharmacy Resolution/Assistance: Resolve pharmacy questions and concerns from patients. - Prescription Verification: Verify the successful electronic transmission of prescriptions, especially SUD medications, to the patient's preferred pharmacy. - Pharmacy Communication: Serve as the central point of contact for pharmacies regarding prescription clarification, refill inquiries, and troubleshooting fill delays, escalating clinical questions to the appropriate staff.
