• Empower patients to manage chronic illnesses • Conduct virtual consultations focused on patient education • Follow partner workflows to review and respond to patient data • Document patient communication in records • Assist patients in setting up their devices • Collaborate with team members as needed • Manage time effectively with excellent organizational skills • Engage with patients and learn new topics in lifestyle medicine

• Develop and maintain individualized Care Plans for management of patients. • Conduct a detailed review of EMR records. • Implement and coordinate all care management activities relating to patients across the continuum of care. • Communicate with the patient’s clinical care team. • Monitor patient progress toward desired outcomes through assessment and evaluation. • Maintain accurate and detailed documentation related to medication, problems, goals, interventions, preventative care, etc. • Conduct monthly care calls to the patients enrolled in the program. • Address all needs/concerns including any follow up needed for patients. • Educate patients on chronic conditions and reinforce the provider’s directions related to chronic conditions. • Evaluate and address social determinants to health and begin to link community resources; then, enter them in the system.

• Reporting directly to the Director of Revenue Cycle Management. • Handling pre-certifications, authorizations, retro-authorizations, appeals, medical records requests, and chart auditing duties. • Partner closely with clinical and revenue cycle teams to remove administrative barriers. • Conduct live reviews with payors and level of care chart reviews. • Clinically negotiate authorization outcomes with the payor. • Provide guidance and training to clinicians on completing P2P reviews. • Timely completion of the Denial Notification process.

• Handle incoming requests via multiple channels (i.e. smart phone, email, etc.) • Primary point of contact for the client • Manage all booking needs for travelers, both business and leisure • Provide exceptional customer service to exceed clients' expectations • Access to sell package options, tours, event ticket and more
