The Opportunity We're seeking board-eligible Endocrinologists to provide comprehensive, empathetic, and collaborative care to women, including diagnosis, treatment, and management of chronic and acute conditions. Requirements - 1+ years of experience as a licensed Nurse Practitioner - Active/clear Nurse Practitioner license to practice in the State of Arizona - Active/Clear DEA - 1+ years of experience working in a rehab setting /rehabilitation facility centers - Strong knowledge in HIPAA compliance as it pertains to telehealth Key Responsibilities - Serve as the operational lead for the interdisciplinary care team. - Lead care coordination efforts for complex Medicaid populations. - Practice autonomously with prescribing authority. - Manage care delivery primarily through telehealth and remote coordination. - Coordinate care across multiple providers, including primary care physicians and health plan case managers. - Support proactive care management, including risk stratification and early detection of health declines. - Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits. - Support patients during hospitalizations and post-discharge transitions. - Help build and shape a new, innovative value-based care model. Schedule and Compensation - Schedule: Primarily a virtual care model with collaborative interdisciplinary teams. - Compensation: The base salary can range from $130,000 to $175,000, depending on experience and whether the role is a Leader position.
