This position involves managing a consistent therapy caseload, guiding clients through treatment goals, and maintaining strong clinical records in a telehealth environment. The work is centered on practical support, sound clinical judgment, and helping clients move forward with measurable progress. Responsibilities - Deliver telehealth counseling and psychotherapy services - Perform initial assessments and gather relevant clinical history - Develop treatment plans based on presenting concerns and goals - Maintain complete, timely, and compliant clinical documentation - Evaluate client progress throughout the course of treatment - Refer clients to additional services or resources when appropriate - Uphold ethical care standards and confidentiality requirements
Job Requirements
- Active New York license as LMFT, LCSW, or LMHC
- Master’s degree in a behavioral health or counseling-related field
- Experience in therapy, counseling, or outpatient mental health care preferred
- Strong organizational and time management skills
- Ability to build rapport and maintain clinical effectiveness in a virtual setting
- Proficiency with telehealth tools and EHR systems preferred

We’re working to deliver exercise and wellness for the older adult cardiovascular disease using telemedicine. We are dedicated to delivering exceptional services that enhance the lives of our patients. Position Overview: The Remote Exercise Physiologist plays a pivotal role in supporting patients enrolled in our cardiac rehabilitation program. The role focuses on providing exercise programming, as well as helping patients navigate through the program through goal setting, progression tracking, and providing education and guidance on heart-healthy lifestyles. The Remote Exercise Physiologist will also support patients in managing their recovery post-cardiac event. The Remote Exercise Physiologist will work closely with a multidisciplinary team including nurses, exercise physiologists, and dietitians to provide comprehensive care for patients in various stages of recovery.
