Job Description As a scout, you will create written and social media content. You will attend high school and club games, watch game film, and publish player-driven analysis on Prep Network's website. You will utilize your existing network with high school and college coaches in your community to grow the Prep Network footprint. We are a rapidly growing sports-focused company where your voice will be heard and you can make a real impact. JOB SUMMARY: The Forecourt Brand Initiative Leader is responsible for leading the improvement of our Commercial Fueling Experience for our Fleet Customers. This role owns the improvement and development of best-in-class experience for our CFL customers. This leader partners cross-functionally to influence the direction and execution of training, communications, operations, and ensuring alignment between strategy and field execution. ***This position will require the individual to work remote with 75% travel within our Commercial Fueling network with a heavy abundance in Michigan, Ohio, the Northeast and Texas.*** KEY DUTIES AND RESPONSIBILITIES: - Strategy - Design, develop, and execute a system-wide, field-based implementation framework to help make sure our CFL experience is best in Class. - Organization Leadership - Own the SOP creation, partner with SSC leadership, and assisting in leading the execution of the developed material. - Change Management - Lead the overall change management strategy and field communications plan to support adoption and sustained execution. This is a work-from-home telephonic Registered Nurse position. As a Care Manager, Telephonic Nurse, you will report directly to the Manager, Care Management. Essential Functions: - Be a CenterWell Home Health representative in supporting patients who have been discharged from service or for those who may need post-acute services. Navigate healthcare options; care services post-acute offerings, Medicare coverage, billing issues, and accessing healthcare resources. - Use a variety of tools and methods to quickly provide patient options and education including but not limited to sites of service, specialty offerings, post-acute care, and other related questions. Handle a variety of customer issues including location lookup, directions, and complaints. - Make clinical level of care determination based on discussion, medical records, and any other important clinical data. Match these needs to a service site location or, if not available, look up and provide alternative services. Be a customer advocate throughout the referral process to ensure timely response and to maximize referral to admission conversion rate.
