The Role The Complex Care Navigator RN is a vital leader in delivering comprehensive, person-centered care to individuals with serious illness. This role involves administering and interpreting a variety of assessments and screening tools to identify and prioritize member needs. As a key member of the multidisciplinary team, the RN is responsible for developing individualized care plans and leading care coordination efforts through active participation in internal and joint rounds. Utilizing Tuesday Health’s electronic systems and tools, the RN ensures a streamlined, technology-enabled approach to care. Central to this role is building strong, empathetic relationships with members, driving collaboration and transforming the member experience. The Role: The Licensed Clinical Social Worker (LCSW) will collaborate closely with an interdisciplinary clinical team to deliver comprehensive, patient-centered palliative care to individuals with serious illness. In this value-based care model, the LCSW plays a critical role in addressing the psychosocial, emotional, and practical needs of patients and their families, supporting care coordination, goal-aligned decision-making including advance care planning and serious illness conversations, and improved quality of life across the continuum of care. Key responsibilities of this role may include: - Conduct comprehensive assessments, including evaluation of psychosocial distress, mental health needs, caregiver burden, social determinants of health, and risk factors impacting patients with serious illness. - Clinically interpret assessment and screening results to identify priority needs, emerging risks, and appropriate interventions within the palliative care model. - Conduct serious illness conversations with patients and families and facilitate advance care planning conversations and documentation - Provide clinical recommendations and escalation to physicians, advanced practice providers, and other clinicians in accordance with established escalation protocols, including concerns related to safety, mental health, caregiver stress, or complex psychosocial needs. - Develop, document, and contribute clinically appropriate interventions within individualized care plans, ensuring alignment with patient goals, values, and value-based care outcomes. - Participate in and contribute clinical insight during interdisciplinary rounds and case conferences, supporting coordinated, goal-aligned care and an optimal patient and family experience. - Deliver short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers coping with serious illness, illness progression, and end-of-life considerations, within scope of LCSW licensure. - Serve as a clinical resource to the care team by identifying psychosocial barriers to care, recommending evidence-based interventions, and supporting transitions across care settings. - Utilize Tuesday Health’s electronic systems, tools, and technology to document clinical assessments, interventions, and care plans, supporting coordinated care delivery and quality measurement in a value-based environment. - Establish and maintain therapeutic, trust-based relationships with patients and families, grounded in clinical listening, empathy, and culturally responsive care. - Collaborate with the multidisciplinary team to improve quality of life, reduce avoidable utilization, and support holistic outcomes including appropriate and timely hospice transitions for members with serious illness. - Support the interdisciplinary team through care team debriefings following emotionally charged cases and the facilitation of periodic memorial rounds for the team A strong candidate will demonstrate the following: - Master’s in Social Work (MSW) from an accredited program - Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the applicable state(s). - Clinical experience in palliative care or hospice is strongly preferred, including experience with advance care planning and serious illness conversations - Experience working within an interdisciplinary care model, collaborating with physicians, advanced practice providers, nurses, care managers, health plan partners, and community-based resources. - Demonstrated ability to exercise independent clinical judgment, including knowing when and how to appropriately escalate concerns or respond in urgent or emergent situations. - Strong verbal and written communication skills, with the ability to clearly document clinical assessments, interventions, and care plans. - Excellent organizational and time-management skills, with the ability to manage multiple priorities while maintaining a high-quality patient and family experience. - Proficiency in using electronic health records (EHRs), clinical documentation systems, and digital tools, with the ability to adapt quickly to new technologies as the organization evolves. - Ability to work effectively in a fast-paced, evolving environment, demonstrating flexibility, resilience, and comfort with change and ambiguity. - Commitment to delivering compassionate, patient-centered care aligned with the goals of a value-based care model.
