As a Health Coach, you will play a vital role in achieving this goal by providing guidance, support, and education to our patients. - Conduct one-on-one health coaching sessions with patients to discuss and develop strategies for lifestyle changes, such as exercise, nutrition, stress management, and smoking cessation. - Provide motivational support and accountability to patients to help them achieve their health goals. - Collaborate with healthcare providers to develop personalized treatment plans for patients. - Assist patients in understanding and managing their chronic conditions, such as diabetes, hypertension, and obesity. - Educate patients on healthy behaviors and provide resources and tools for them to make informed choices about their health. - Stay current with the latest health and wellness research and trends to incorporate into patient care. - Maintain accurate and detailed records of patients' progress and provide regular reports to healthcare providers. Qualifications - Bachelor's degree in Health Education, Nutrition, or a related field. - Certification as a Health Coach or Wellness Coach preferred. - Minimum of 2 years of experience in health coaching, patient education, or related field. - Knowledge of behavior change theories and techniques. - Excellent communication and interpersonal skills. - Ability to work independently and as part of a team. - Strong organizational and time-management skills.
