• Provide specialist pharmaceutical support to patients supported via NHS Partners • Carry out reviews for patients with long term conditions within clinical competencies • Collect and record patients’ health data in a holistic manner • Interpret, risk stratify, and manage health data in line with standard operating procedures and NICE guidelines • Provide evidence-based lifestyle advice • Prescribe/alter medications for patients with long term conditions • Maintain regard for patient safety and dignity • Build and maintain trust with patients • Maintain contemporaneous clinical notes within Suvera’s and GP Partners systems • Be proactively involved in Quality Outcomes Framework (QOF) • Undertake other administrative duties where reasonably required

• Carrying out reviews for patients with long term conditions within your clinical competencies and providing specialist support to patients that Suvera is supporting via our NHS Partners. • Work across multiple partner organisations, the role will have a predominantly patient facing element. • Including chronic disease management, repeat prescription management and structured medication reviews • Collecting and recording patients’ health data in a holistic manner • Interpretation, Risk stratification, and Management of this data in line with standard operating procedures and NICE guidelines • Able to provide evidence based lifestyle advice • Prescribing / altering medications for patients with LTCs • Maintaining at all times a regard for patient safety and dignity • Building and maintaining trust with patients • Maintaining contemporaneous clinical notes within Suvera’s and GP Partners systems • Being proactively involved in Quality Outcomes Framework (QOF) • Undertaking other administrative duties where reasonably required

• Provide specialist pharmaceutical support to patients in collaboration with NHS Partners • Conduct reviews for patients with long-term conditions within clinical competencies • Manage chronic disease management and structured medication reviews • Actively participate in quality improvement and audits • Collect and record patients’ health data holistically • Support medicines management and monitoring as per guidelines • Offer evidence-based lifestyle advice and prescribing medications
