Role Description Are you a GP looking for meaningful, flexible clinical work — without the commute? Veteran Pathways & Proactive Care is seeking experienced General Practitioners for a fully remote telehealth position, delivering structured case conferencing and chronic disease management for veterans and patients with complex, multi-system health needs. This is not a standard telehealth locum. You will provide clinical leadership within integrated Chronic Care Teams, working collaboratively with allied health professionals and specialists to translate clinical findings into coordinated, evidence-based care plans. The work is structured, purposeful, and genuinely multidisciplinary. We work across both Medicare and DVA billing streams, with a dedicated focus on veteran health and chronic disease management. Qualifications - Medical degree (MBBS or equivalent) - Current registration with the Medical Board of Australia (AHPRA) - Fellowship with RACGP or ACRRM (VR GP), or eligibility for 19AB exemption in a DPA location (Central Coast, Newcastle) - Experience in chronic disease management, preventative health, and complex care coordination - Competence in prescribing, referrals, and investigation management - Strong communication and collaboration skills within multidisciplinary teams - A suitable home-office environment and reliable internet connection for telehealth delivery Requirements - Lead and participate in scheduled telehealth case conferences as part of multidisciplinary Chronic Care Teams - Provide medical oversight and clinical direction for patients with complex chronic and multi-system conditions - Integrate findings from clinical assessments, pathology, imaging, and allied health input into coordinated, evidence-based care plans - Undertake chronic disease management (CDM) reviews and care planning, including GP Management Plans (GPCCMPs) and Team Care Arrangements (TCAs) - Collaborate with allied health professionals, specialists, and care coordinators to optimise patient outcomes - Deliver telehealth consultations for patient follow-up, clinical review, and ongoing management - Manage prescribing, referrals, and investigation requests as part of the coordinated care model - Ensure clinical safety, prioritisation, and escalation pathways are clearly identified and actioned Benefits - Guaranteed Hourly Rate — $240/hr for the first 3 months, moving to 70% of billings thereafter - Sign-On & Anniversary Bonuses — Financial incentives available to support your transition (details discussed at interview) - Work From Home — Fully remote telehealth position with no commute and no physical clinic attendance required - Flexible Scheduling — Structured session blocks that can be arranged to suit your availability - Meaningful, Purpose-Driven Work — Deliver coordinated care that genuinely improves outcomes for veterans and patients with complex chronic conditions - Multidisciplinary Collaboration — Work within established Chronic Care Teams alongside allied health professionals and specialists - DPA Location Benefits — GPs on the Central Coast or in Newcastle can access DPA pathways, including 19AB exemption eligibility - Structured Clinical Model — Be part of a system that values thorough, evidence-based care over high-volume throughput - Full Support Provided — Administrative, nursing, and allied health support so you can focus on clinical care
