Role Description Ortus Health provides remote medical coding, pharmacy, and GP triage services to NHS GP practices across the UK. We're looking for a Remote Care Coordinator & Administrator to support our remote triage service across the practices we work with. This is a genuinely hybrid role and it's central to keeping patient triage moving smoothly between practices and our clinical team. You'll be the person making sure triage tickets are tracked, accurately recorded, and routed to the right GP, and that patients receive and complete the questionnaires our clinicians need ahead of review. Your focus is coordination, accuracy, and keeping the workflow on track. The role is fully remote and full-time, and we supply all equipment. Responsibilities - Monitor and manage the volume of incoming triage tickets across the GP practices we support, ensuring each is actioned within agreed timeframes. - Accurately record and update information and triage data across clinical systems (EMIS, SystmOne, Docman). - Route and reassign triage tickets to the appropriate GP. - Send patient questionnaires, track their completion, and ensure they're ready for GP review. - Maintain accurate records and documentation in line with information governance standards. - Provide general administrative support across the Ortus services. - Liaise with practice staff and our clinical team to keep workflows running smoothly and flag issues early. Qualifications - Prior experience in a GP practice is essential. - Hands on experience with clinical systems. - Strong organisational skills and the ability to manage a high volume of tasks accurately in a remote setting. - Excellent written and verbal communication, for working with both patients and healthcare professionals. - Confident with computers and office software. - Self-motivated and proactive, with the ability to work well within a remote team. - A calm, accurate, detail-focused approach — much of this role is about making sure nothing slips through the cracks. Requirements - The right to work in the UK. - An enhanced DBS check (we will arrange this). - Completion of data security and information-governance training (provided by us). - A private, secure space to work from home. Role Description Ortus Health is seeking a skilled Clinical Pharmacist – Medicines Reconciliation to join our dynamic team in a fully remote capacity. In this role, you will play a vital part in ensuring medication safety and continuity of care by completing medicines reconciliations from clinical correspondence, including hospital discharge summaries and outpatient clinic letters. Working as part of our remote multidisciplinary team, you will review and reconcile changes to patients’ medication regimens. This includes assessing for discrepancies, potential drug interactions, and ensuring that any updates are accurately actioned within the patient record. Key Responsibilities - Complete thorough medicines reconciliations using clinical correspondence, including hospital discharge summaries and outpatient letters. - Identify and resolve discrepancies between current medication records and updated hospital communications. - Review and document medication changes, initiating appropriate actions or flagging for GP follow-up where required. - Collaborate remotely with GPs, care coordinators, and practice teams to support safe and accurate prescribing. - Use clinical systems and decision-support tools to check for potential drug interactions, duplications, or contraindications. - Maintain accurate clinical documentation in line with practice protocols and professional standards. - Engage in ongoing professional development to ensure up-to-date knowledge of medicines optimisation and primary care best practice. Qualifications - Must be a registered pharmacist with GPhC registration. - Must be an independent prescriber. - Must have completed CPPE pathway. - Excellent analytical and problem-solving skills with attention to detail. - Effective verbal and written communication abilities for patient and team interactions. - Demonstrated ability to work independently and manage multiple tasks remotely. Role Description We are seeking experienced and well-organised GPs to join our remote triage team. As a Triage GP with Ortus Health, you will make clinical decisions that directly influence patient outcomes. You’ll be part of a dynamic, supportive team, working remotely but closely connected to the practices we serve. - Demonstrate clinical leadership and strong organisational skills - Help meet practice clinical governance frameworks and keep protocols up to date - Liaise with practices to look into complaints or queries - Interviewing, onboarding and training new clinical staff - Triage incoming patient contacts from multiple practices using digital platforms - Make safe and effective clinical decisions based on limited information when required - Provide telephone consultations as needed - Issue prescriptions, fit notes, referrals, and safety-netting advice - Escalate complex or high-risk cases appropriately - Support the wider remote team, including pharmacists and care coordinators - Adapt to different clinical systems (e.g., EMIS, SystmOne) and local processes - Maintain clinical oversight and “keep your eye on the ball” across multiple patient flows - Accurately document consultations in line with GMC and practice standards - Audit the work of the rest of the team, including other GPs, pharmacists and care coordinators Qualifications - Full GMC registration with a license to practise - NHS England GP Performers List inclusion - At least 2 years post-CCT (preferred) - Experience with triage and remote consultations - Ability to manage clinical risk safely in a remote environment - Familiarity with EMIS, SystmOne, or other GP clinical systems like Accurx, Patchs, eConsult etc - Excellent time management and attention to detail - Able to remain focused and clinically vigilant while managing multiple practice workflows - Strong communication skills and a collaborative approach Benefits - Competitive hourly rates - Inclusive work from home role - Supportive IT infrastructure and administrative assistance - An opportunity to gain experience in a unique, remote setting
