Role Description The Home Care Coordinator (HCC) processes home care referrals and submits clinical data to the home care agencies. The HCC compiles clinical information from the EMR to send to the home care agency to facilitate the transition of care. The HCC acts as a liaison with the inpatient staff, physician offices, and home care agency to ensure the clear communication of the home care orders and the patients’ medical needs. Qualifications - Associate's Degree in Health Care, Business, or related field (Required) - 1 year of customer service experience required, preferably in the healthcare industry (Required) - Experience entering data, navigating, and retrieving information from an electronic medical record (EMR), i.e. Epic, Careport Care Management (Required) - Experience in home care placements and/or home care intake highly desirable (Preferred) - Knowledge of managed care Medicare, Medicaid, or other insurance authorization coupled with understanding of CMS regulations related to homecare services preferred - Knowledge of home care insurance authorization process preferred - Excellent verbal and written communication skills (Required) - Strong customer service orientation (Required) - Ability to adapt to change and work in a team-based environment (Required) - Ability to manage multiple priorities (Required) Requirements - Processes preadmission and inpatient referrals in Epic status board, in addition to coordinating services for international patients - Coordinates out of area home care referrals via Careport Care Management system to ensure timely and appropriate start of care in accordance with physician orders - Verifies that all required clinical documentation is accurately attached and submitted to the designated agencies - Determines and processes in-depth knowledge of home care providers, insurance coverage requirements, and agency service areas - Coordinates and obtains face-to-face encounter forms on home care referrals - Arranges home labs for patients as appropriate - Responds to all electronic platform, phones, email, Epic status board reports to ensure home care referral requests are processed and documented - Responds to inbound phone calls and determines customer needs while providing accurate and responsive intake and ensuring customer satisfaction - Responds to and resolves non-clinical issues and complaints in an accurate, effective, and timely manner - Documents all home care referrals in Careport Care Management and Epic - Fax-attaches required documentation as needed in Careport - Oversees home care phone line, answers and responds to inquiries, and assists with urgent staff and patient issues - Maintains satisfactory attendance record - Reports for duty punctually - Adjusts to changing situations and work assignments Benefits - The base pay scale for this position is $26.65 - $40.87 - This position will be eligible for additional benefits consistent with the role - The salary of the finalist selected for this role will be determined based on various factors, including but not limited to: scope of role, level of experience, education, accomplishments, internal equity, budget, and subject to Fair Market Value evaluation - The hiring range listed is a good faith determination of potential compensation at the time of this job advertisement and may be modified in the future Company Description HSS is consistently among the top-ranked hospitals for orthopedics and rheumatology by U.S. News & World Report.
