• Conduct housing assessments and determine client eligibility for housing caseload of 20 clients per month in person. • Assist clients with housing search, applications, and move-in coordination • Build relationships with landlords and local housing programs • Support clients in maintaining lease compliance and resolving tenancy issues • Provide ongoing tenancy-sustaining services (budgeting, life skills, advocacy) • Collaborate with MCO housing teams and Grace Community Care’s intake and billing departments • Maintain accurate documentation and case notes in Microsoft. • Participate in regular team meetings and training

• Conduct comprehensive biopsychosocial assessments • Develop individualized, measurable treatment plans • Provide individual, family, and/or group therapy • Deliver evidence-based interventions including: - Cognitive Behavioral Therapy (CBT) - Trauma-informed therapy - Motivational Interviewing - Psychodynamic therapy - Behavioral interventions • Provide crisis intervention and safety planning as needed • Maintain timely and accurate progress notes • Complete assessments and treatment plans per Medicaid guidelines • Utilize appropriate CPT and ICD-10 coding • Maintain compliance with HIPAA and NJ Medicaid standards • Use Electronic Medical Records (EMR) systems for documentation • Collaborate with multidisciplinary care teams • Participate in case consultations as needed • Coordinate services with housing support and community providers • Support continuity of care across service settings

• Post job openings on recruitment platforms including Indeed, LinkedIn, nonprofit job boards, and social service hiring platforms • Screen resumes and evaluate candidate qualifications • Conduct first-round screening interviews to assess candidate experience, communication skills, and job readiness • Coordinate interview scheduling with program leadership • Maintain an organized recruitment pipeline and applicant tracking system • Communicate with candidates throughout the hiring process • Provide leadership with recommended candidates for final interviews • Assist with identifying staffing needs across housing, healthcare, and IDD programs • Support recruitment strategies to attract candidates with experience in case management, social services, healthcare, and disability services • Track hiring activity and recruitment metrics • Maintain strong candidate communication to ensure a positive hiring experience • Prepare and send employment offer letters • Coordinate background checks and employment verification • Assist with new hire onboarding documentation • Ensure all required hiring paperwork is completed and properly filed • Maintain employee personnel records and documentation • Ensure HR records remain organized and compliant with internal policies • Support HR administrative tasks related to employee onboarding and workforce management

• Conduct orientation sessions for new employees • Introduce staff to organizational policies, programs, and service expectations • Ensure all onboarding steps are completed for new hires • Support new employees as they transition into their roles • Manage and maintain training modules within the LMS platform • Assign required courses to new employees • Monitor and track staff training completion • Update training materials as program policies evolve • Provide training on housing stabilization services and program expectations • Teach documentation standards and service note requirements • Support staff understanding of Medicaid service documentation • Coordinate training related to client engagement, housing search strategies, and community resource navigation • Coordinate shadowing opportunities for new staff • Support practical learning experiences • Provide coaching to staff during their first 30–60 days • Work closely with supervisors to ensure new staff develop strong service delivery skills • Facilitate periodic staff training sessions • Identify skill gaps and training opportunities • Recommend improvements to training systems and onboarding processes • Support ongoing professional development for staff
