The department offers comprehensive diagnostic and interventional procedures to our patients. The clinical staff is comprised of a mixture of registered nurses, cardiovascular technologists, and radiologic technologists. The department is comprised of two Cath Labs, one EP Lab, and a six bed holding room for pre- and post-procedure care. Additionally, Cath/EP Lab staff work collaboratively with the cardiac surgery team in the hybrid CVOR to care for patients undergoing TAVR procedure. Invasive Cardiology staff is cross-trained to care for patients in all of the areas listed above. - Cath Lab Duties Performs daily equipment and room checks to ensure safe operation of all cardiology supplemental equipment. This includes quality assurance checks, logging of results, troubleshooting equipment, and calibration of equipment. Technologist will notify leadership of any equipment requiring a service call by the vendor. Acquires/maintains competency in all diagnostic and interventional cardiac catheterization procedures done at Mount Auburn Hospital. This includes (but not limited to) diagnostic coronary angiography, percutaneous interventions, right heart cath, pericardiocentesis, left ventricular support devices (intra-aortic balloon pump, Impella, and extracorporeal membrane oxygenation). The technologist will acquire/maintain competency in aseptic and sterile techniques for the preparation and maintenance of procedural equipment and sterile table. The technologist will acquire/maintain competency in hemodynamic monitoring. Hemodynamic monitoring includes (but not limited to) continuous arterial pressure and right heart pressure monitoring. Technologist will communicate any acute changes to the physician and nurse. Acquires/maintains competency in various procedure room roles as needed for the department (circulating, monitoring, and/or scrubbing). Circulator role: The circulator will gather and hand-off all necessary diagnostic and emergency equipment to the physician and/or scrub. Monitor role: The monitor will analyze and monitor the patient’s heart rate, heart rhythm, and invasive blood pressures throughout the duration of the procedure and alert the physician and team of any acute changes. The monitor is also responsible for observing and documenting all the pertinent procedure details in the patient’s electronic medical record. Scrub role: The scrub will don sterile attire (gown and gloves) in order to directly assist the attending physician throughout the duration of the procedure. The scrub will maintain competency in all necessary diagnostic and interventional equipment on the sterile field. The technologist will maintain competency in standard cath lab equipment: diagnostic catheters, guidewires, interventional catheters, pulmonary artery catheters, angioplasty balloons, drug-eluting stents, bare-metal stents, and arterial closure devices. The technologist will acquire/maintain competency in the usage of Rotapro (formerly known as Rotablator) for coronary rotational atherectomy, IVUS (intra-vascular ultrasound), and FFR (fractional flow reserve). The technologist will successfully complete and maintain competency in AHA ACLS (American Heart Association Advanced Cardiovascular Life Support) and resuscitation teamwork through training and classwork. ACLS certification will be renewed every 2 years. Acquires/maintains competency in the initiation of mechanical circulatory support for hemodynamic support: intra-aortic balloon pump (IABP), Impella, and extracorporeal membrane oxygenation. EP (Electrophysiology) Lab Duties The technologist will acquires/maintain competency in all diagnostic and interventional EP procedures done at Mount Auburn Hospital: diagnostic electrophysiology and pacing maneuvers, supraventricular tachycardia ablations, atrial flutter ablations, atrial fibrillation ablation (pulmonary vein isolation), atrioventricular node ablation, ventricular tachycardia ablations. The technologist will acquire/maintain competency in the workflow of utilizing the cardiac mapping system during diagnostic and interventional electrophysiologic procedures. The technologist will be responsible for monitoring the patient’s heart rate and rhythm during all EP procedures and immediately notifying the physician of any acute changes or life-threatening rhythms. The technologist will be competent in emergency defibrillation and emergency anti-tachycardic pacing maneuvers. The technologist will acquire/maintain competency in the set-up and usage of ablation catheters and ablation generators. The technologist will acquire/maintain competency in all cardiac rhythm management (CRM) device implants and generator change procedures: implantable cardioverter-defibrillator (ICD) and pacemakers. The technologist will acquire/maintain competency in sterile technique required for pacer and ICD implant procedures. The technologist will also maintain full competency in the scrub role during implants and assisting the physicians. CVOR (Cardiovascular Operating Room) Duties The Cath/EP technologist will be assigned to assist the physician and team with structural heart procedures in the CVOR: transcatheter aortic valve replacement (TAVR), patent foramen ovale (PFO) closures, left atrial appending closures (Watchman device implants), or other procedures as assigned. The technologist may be assigned to any of the following roles in the CVOR: scrub tech, circulating tech, or recording tech. The technologist will be competent in all roles. Collaborates with CVOR and Perfusion staff for continuity of care during the cases noted above or as assigned.
