Role Description This is a remote position requiring travel to support enterprise-wide Virtual Health Services. As our Virtual Integrated Care Professional, you will deliver comprehensive, coordinated health services remotely, leveraging technology to connect with patients and facilitate seamless care across various disciplines. Every day you will engage patients through telehealth and other digital tools to conduct assessments, provide education, manage chronic conditions, and coordinate care plans with multidisciplinary teams. To be successful in this role, you must possess strong communication and clinical assessment skills, comfort and proficiency with virtual healthcare technologies, an ability to work autonomously and collaboratively, and a commitment to patient-centered, integrated care. - Serve as a Virtual RN (VIC RN) for 50% of the role, providing direct patient care during implementations including travel. - Virtually collaborate with interdisciplinary teams, assess patient status, and develop comprehensive care plans. - Respond to patient issues, educate patients/families, and advocate for their needs while using technology effectively. - Coach and mentor bedside nurses, focusing on quality, cost containment, and compliance with standards. - Dedicate 50% to Care Transformation: orienting/mentoring VIC RNs, leading simulations and education, and monitoring workflows/technology. - Act as a program leader, providing leadership support, troubleshooting barriers, and guiding staff adjustment based on census. Qualifications - Bachelors of Nursing. - 5 years of overall acute care nursing experience. - 3 years of nursing preceptor experience. - 2 years in a nursing leadership role. - Basic Life Support - CPR. • Conduct IOP orientations for new patients, introducing program structure, expectations, and resources. • Lead discharge planning beginning at admission, ensuring linkage to outpatient providers, community resources, and payor-approved step-down services. • Document collateral contacts, discharge coordination, and case management activities in compliance with CMS and commercial payor standards. • Facilitate psychotherapy, process and skills-based groups (e.g., DBT skills, relapse prevention). • Participate in weekly case conferences, contributing psychosocial and systems-level insights to treatment planning. • Track engagement, discharge, and readmission metrics; report trends to the Clinic IOP Director for quality-improvement initiatives. • Assist in updating safety plans and activating crisis escalation pathways as needed.

• Deliver individual and group psychotherapy services to patients in outpatient settings, utilizing empirically supported treatment approaches. • Collaborate with a multidisciplinary treatment team to formulate and execute individualized treatment plans. • Maintain accurate and timely clinical documentation in strict compliance with all legal, ethical, and organizational standards. • Participate actively in scheduled clinical supervision, consultation sessions, and team meetings. • Adhere to structured therapy protocols and programmatic models of care to ensure clinical consistency and excellence. • Respond effectively to clinical crises and strictly observe established risk-management procedures.

• Responsible for the overall clinical, technical and administrative functions at the location regarding the PAP Therapy program • Responsible for installation/setup of respiratory therapy equipment (CPAP, AutoPAP, BiPAP) and provision of supplies • Ongoing assessment of patients and equipment • Schedule setups and explain clinical benefits of PAP therapy • Maintain records, charting and documentation • Ensure updated demographic information • Oversee regulatory rules for respiratory therapist staff • Train and orient new respiratory staff • Effective written and verbal communication skills • Work with office staff on billing and documentation • Promote home respiratory therapist services • Ensure safe operation of all equipment • Availability for patient contact and response to needs
