Care Management Support Coordinator III – NICU

• Works with care management team on administrative care management activities • Performs outreach, answers inbound calls, and schedules services • Serves as a point of contact to members, providers, and staff to resolve issues • Documents member records in accordance with current state and regulatory guidelines • Provides outreach to members via phone to support with care plan next steps • Connects members to community or health plan resources • Supports member onboarding and day-to-day administrative duties • Works with care management team on escalating requests and inquiries to management • Complies with all policies and standards
Job Requirements
- Requires a High School diploma or GED
- Requires 2 - 4 years of related experience
