• Receive new patient referrals from clients • Create charts for all new patients • Input medical information in new chart • Help Care Coordinators with patient needs as applicable • Foster relationships with their team to ensure the CareHarmony culture is positive and supportive. • Other duties as applies

• Understand and adhere to the National EMR Policies and Procedures Manual. • Collect, enter, and ensure documentation for medical staff at designated site(s). • Perform all administrative duties of the clinic or designated site(s). • Complete accurate documentation of activities performed. • Communicate effectively with health care providers regarding care for service population. • Maintain adequate inventory of all medications/supplies.
