Community Health Program Coordinator

Role Description As a CHW Program Coordinator, you are the operational engine behind our CHW program in Georgia. You connect eligible patients to life-changing care, keep data clean and actionable, and ensure the program runs smoothly — even when things get complicated. You work closely with CHWs, Field Managers, and clinical partners, bridging the gap between people and the systems that support them. Qualifications - 1–3 years of experience in program coordination, research administration, or equivalent - Demonstrated experience with data collection, analysis, and protocol implementation - Proficiency in Microsoft Excel (reporting, data organization, pivot tables) - Strong written and verbal communication skills - Excellent organizational skills and keen attention to detail - Ability to work independently and solve problems effectively - Flexibility and adaptability in a fast-paced environment - Ability to work on-site in Atlanta, GA Requirements - Bachelor's Degree in Public Health, Social Work, Healthcare Administration, or a related field (preferred) - Experience with an EMR system such as EPIC or case management platforms (preferred) - Familiarity with SDOH screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) (preferred) - Experience with program or research enrollment workflows (preferred) - Knowledge of Medicaid, D-SNP, or public benefits programs (preferred) - Comfort working in community health, social services, or healthcare settings (preferred) Benefits - Salary: Based on experience and location - Medical, dental, and vision insurance - 401(k) plan - Paid time off and parental leave - Employer-provided life insurance - Employee Assistance Program (EAP) - Professional development assistance - Mileage reimbursement for work-related travel Company Description IMPaCT Care is an Equal Opportunity Employer.
