Role Description Enrollment is the entry point to care—and a strong first conversation sets everything that follows. As a Care Access Specialist, you’re the patient’s introduction to KangarooHealth. You’ll enroll patients into both our Remote Patient Monitoring and Chronic Care Management programs, answer real questions, secure consent, and guide patients through onboarding. For CCM patients, you’ll also complete the first care call—ensuring they feel supported and connected from day one. This is not a scripted sales role. Patients need to trust you, not feel sold to. You earn that trust in the first call—by being honest, clear, and genuinely invested in helping them access care that improves their life. What You’ll Do - Make outbound calls to eligible RPM and CCM patients on behalf of KangarooHealth’s partner provider groups - Explain our Remote Patient Monitoring and Chronic Care Management programs clearly and compassionately—what they are, why they matter, and what patients can expect - Address skepticism and concerns directly and truthfully—no scripts, no pressure - Secure verbal consent and complete patient enrollment in KangarooHealth’s system - Meet daily enrollment targets while maintaining quality and accuracy on every call - Conduct the first CCM care call with newly enrolled patients, covering care plan review, chronic condition education, and patient goals - Provide health coaching and education on chronic condition self-management (HTN, DM, CHF, COPD) - Identify and escalate clinical concerns or urgent patient needs to the nursing or provider team - Accurately document CCM time and call activity in the client’s EMR per billing protocol - Apply working knowledge of CCM billing codes (99490, 99439, 99487, 99489) to ensure compliant documentation Qualifications - Calm under pressure, clear under stress—you stay steady when a patient is frustrated or confused - You listen before you speak and guide without pushing—patients trust you because you’ve earned it - You can explain a government program to a skeptical senior and gain their confidence in under five minutes - Organized and self-directed—you manage your call queue, hit goals, and don’t need to be micromanaged - Comfortable with volume—you can execute high-volume outreach daily without losing quality or warmth - Tech-fluent—you navigate CRMs, EMRs, spreadsheets, and digital workflows without friction - Accountability-minded—you follow the playbook, flag problems early, and ask smart questions Requirements - Active LPN or LVN license with multi-state compact privileges - 3+ years of clinical experience in a hospital, family practice, or geriatric setting - 3+ years of EMR documentation experience; ability to learn new platforms quickly - 1+ year of remote work experience with patient phone engagement - Strong understanding of chronic conditions and their management (HTN, DM, CHF, COPD) - Familiarity with Medicare, CCM/RPM program requirements, and CMS billing codes - Technical fluency with G-Suite, Microsoft Office, Hubspot, Slack, Zoom, Epic, ECW, Athena, and similar platforms - Reliable high-speed internet, dedicated computer, and a private, HIPAA-compliant home workspace - Available to work 40 hours per week aligned with client clinic hours (9AM–6PM local time) Preferred Experience - Experience with CCM, RPM, or other value-based care programs - Prior call center or high-volume phone outreach experience - Medical Assistant certification (CMA, RMA, CCMA) or equivalent healthcare credential - Bilingual fluency in Spanish or another language (a plus, not required) Compensation & Benefits - $55,000–$65,000 total compensation - Base salary plus performance-based incentives bonus - Fully remote — no commute, no relocation - Medical, Dental, and Vision insurance - FSA, Life and AD&D, 401(k) - Paid time off - Clear growth path within a fast-scaling digital health company
