Role Description The Patient Intake & Scheduling Coordinator serves as the first point of contact for new and prospective patients of the client. This role is responsible for managing inbound and outbound patient communications, collecting accurate demographic and insurance information, coordinating with internal verification teams and physical therapists, and ensuring timely and accurate scheduling of patient evaluations and follow-up appointments. The position plays a critical role in delivering an excellent patient experience while ensuring no leads or patients fall through the cracks. - Answer inbound calls from new patients and respond professionally to inquiries regarding services and appointments - Conduct outbound calls to new patient leads and follow up on online bookings to obtain any missing or required information - Collect complete and accurate patient demographic and insurance information - Obtain insurance cards from patients and verify acceptance prior to scheduling - Coordinate closely with the insurance verification team to ensure patients are booked appropriately based on coverage and plan acceptance - Collaborate with Physical Therapists (PTs) to follow up on patients requiring scheduling or rescheduling - Schedule patient evaluations and follow-up appointments up to 4–6 weeks in advance - Create and maintain patient charts with a high level of accuracy and attention to detail - Ensure all patient data is documented correctly to support clinical and billing workflows - Maintain organized tracking of patient status to prevent missed follow-ups or dropped leads Qualifications - Excellent customer service and patient communication skills - Strong verbal and written English skills for daily interaction with patients and internal teams - Proven ability to multitask in a fast-paced, patient-facing environment - Strong organizational skills to ensure timely follow-ups and accurate scheduling - High attention to detail and commitment to accuracy in data entry and documentation Requirements - Experience in healthcare intake, patient scheduling, or medical front-desk operations (preferred) - Familiarity with insurance verification processes (preferred) - Experience working with EMR or patient management systems (preferred) - High school diploma or equivalent required - Healthcare administration or related coursework is an advantage Benefits - HMO with 1 free dependent upon hire - Life Insurance - Night Differential - 20 PTO credits annually - VL and SL cash conversion - Annual Performance-Based Merit Increases and Employee Recognition - Great Company Culture - Career Growth and Learning - A laptop will be provided by the company - NIGHT SHIFT - WORK FROM HOME - Php 30,000 - 34,500 all in

• Contact individuals or couples interested in Hand to Hold's counseling services to complete intake paperwork and coordinate scheduling for therapy sessions • Conduct necessary assessments and work in partnership with clients to define their treatment goals • Provide short-term treatment using appropriate therapy modalities; counseling sessions are virtual using a telehealth system • Track and report program data within program timelines to collect demographic information and measure impact, satisfaction, and overall program effectiveness • Communicate promptly and regularly with clients and stakeholders • Attend Hand to Hold staff meetings and support programs team as needed • Attend recommended training or professional development opportunities • Submit weekly and monthly reports – program data, timesheets, expense reports – in a timely fashion • Perform other duties as assigned by the Counseling Program Manager

• Under general supervision, the Therapist uses extensive knowledge and skills obtained through education and experience to perform responsible and difficult professional casework. • Provides intensive therapy involving complex social problems and treatment plans. • May provide a wide array of social work functions such as assessment interviews, counseling, or case evaluation. • Determines appropriate methods of treatment and/or provides other social services to individuals, groups, or families. • May also provide referrals to appropriate community services or financial assistance. • Provides accurate and understandable interpretations of program guidelines via a client orientation program as well as maintaining proper records according to organization, state and federal guidelines.

• Under the supervision of the Program Director, the Speech Therapist provides comprehensive speech therapy services to participants • Evaluating participants and developing individualized treatment plans • Deliver therapy and educate families and staff on techniques to support participant progress • Ensure all services are in compliance with Title 22, California Code of Regulations • Work collaboratively with interdisciplinary team members and contribute to care planning • Support the program in achieving its service and quality goals.
