• Review patient intake assessments/charts, and conduct telehealth evaluations to evaluate work-function impairment and medical necessity for Short-term Disability (STD) to attend the Charlie Health program. • Complete, certify, and sign Short-term disability forms with accurate, consistent information. • Coordinate with Charlie Health clinicians and care coordinators to respond to any clarifying requests from Employer contacts/third-party adjudicators to resolve information gaps and support patients’ disability leave, while protecting PHI. • Meet service-level timelines submission of Short-term disability paperwork, including timeliness of extensions. • Maintain HIPAA and state/federal compliance, and contribute to continuous process improvement. • Provide direct patient care and education for medication management, diagnostic evaluations and medical assessments, as needed. • Work diligently with support staff to ensure proper patient communication and follow up, as needed. • This job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. • Monitors crisis queues for clients receiving individual therapy, family therapy, and participating in group sessions • Responds to inbound crisis calls providing real-time risk assessment, de-escalation, and safety planning • Screen risk and collaborates on stabilization planning for clients at risk of harm to themselves or others • Completes all required documentation in alignment with compliance standards and Charlie Health's best practices • Serves as key point of contact for crisis triage during client group sessions in collaboration with Senior Care Coaches and Care Coaches • Conducts case consults with Care Team members as needed • Supports all Care Team members (i.e., Primary Therapists, Care Coaches, Group Facilitators, etc.) in collaboration regarding the client's care • Other care coordination tasks as needed • Other tasks and duties as assigned by the Director of Crisis Response and Prevention or the Chief Clinical Officer
