Job Summary and Qualifications As the Centralized Order Entry Pharmacist, you will ensure that medication orders meet standards for patient care. In this role, you will monitor prescriptions, verify that the dosage and type of medication are appropriate for the patients condition, enter the orders into an online database, track and oversee prescriptions, evaluate patient care plans, and provide any necessary recommendations for the patient. What you will do in this role: Reviews and interprets all physician orders received, using the patient profile Screens for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry Monitors for incompatibilities, concentration and rate on intravenous drugs Prepares written or oral report at change of shift to communicate incomplete assignments and potential patient safety concerns to monitor in order to ensure continuity of medication therapy. Ensures facility procedures are followed when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered Investigates and reports adverse drug events and medication incidents Facilitates dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consulted What qualifications you will need: Bachelor's degree from an accredited college of pharmacy is required. Role Description The incumbent serves as a Pharmacy Lead Technician in a VISN Clinical Contact Center (CCC) as part of an interdisciplinary healthcare team. The Pharmacy Lead Technician in the CCC functions independently under the supervision of the Pharmacy Contact Center Technician Supervisor. Major Duties: - Works collaboratively with VISN programs, services, and interdisciplinary coordinated care team members to provide access to care and meet the needs of Veterans, their family members and/or legal representatives via various available contact modalities including telephone calls, secure messaging, chat, text messaging, and video. - Addresses a broad range of topics, including medication refills/renewals, basic billing inquiries, and procedural pharmacy activities. - Routinely reviews computer medication profiles for therapeutic duplication, prescription non-compliance, and allergies/drug reactions. - Transfers identified patients to a Pharmacist or supervisor for further evaluation, if needed. - Processes and submits patient requests for new prescription orders and renewals (including controlled substances) accurately, thoroughly, and in a timely manner. - Uses approved Pharmacy Service protocols when appropriate to satisfy patient medication requests. - Professionally handles issues escalated from team members and follows guidelines to address the issue appropriately. - Actively and diligently treats all Veterans with respect and utmost dignity. - Seeks to solve Veteran's concerns at first contact and refers to other VA resources when required. - Always resolves all Veteran issues in a timely and caring manner with emphasis on providing first contact resolution.
